脑卒中国际功能残疾和健康分类简要核心组合的信度与效度研究

摘　　要：目的:分析研究脑卒中国际功能、残疾和健康分类(ICF)简要核心组合的信度和效度。方法:选择50例脑卒中患者,采用国际脑卒中ICF简要核心组合、美国国立卫生研究院卒中量表(NIHSS)、Fugl-Meyer运动功能量表(FM)、Barthel指数(BI)进行评定。评定者间信度采用Kappa一致性分析方法;同步效度采用Spearman相关分析。结果:身体功能中的四个类目意识功能、定向功能、语言精神功能和肌肉力量功能的kappa值为0.664—1.000,信度中度到优;身体结构的类目脑的结构kappa值为0.976,信度优;活动和参与的四个类目的kappa值为0.696—0.846,信度中度到优;环境因素的类目直系亲属家庭kappa值为0.450,信度中度。ICF除"e310直系亲属家庭"外的其他项目的总分与NIHSS、Fugl-Meyer、BI评分的Spearman相关系数分别为0.795、-0.866、-0.795(P<0.01)。结论:采用国际脑卒中ICF简要核心组合对脑卒中患者进行综合评定可靠有效。Objective： To analyze and study the inter-rater reliability and validity of international classification of functioning, disability and health（ICF） brief core sets for stroke.Method： Fifty stroke patients were involved in this study. The functional evaluation were assessed by ICF brief core sets for stroke, National Institutes of Health Stroke Scale （NIHSS）, Fugl-Meyer motor scale（FM） and Barthel index （BI）. The inter-rater reliability was analyzed using Kappa correlation statistics. The concurrent criterion va-lidity was analyzed by Spearman rank correlation coefficients.Result： The categories of ICF brief core sets for stroke had moderate to excellent inter-rater reliability （Kappa value 0.450--1.000）. Correlation analyses showed that the total score of brief core sets were highly associated with scores of NIHSS, FM and BI. Spearman＇s rho in NIHSS, FM and BI were 0.795, -0.866 and -0.795, respec-tively （P〈0.01）.Conclusion： The ICF brief core sets for stroke is reliable and valid as a measurement for stroke patients.
