功能性颞下颌关节成形术的初步报告

摘　　要：目的　功能性颞下颌关节成形术是本文设计的新颖术式,缘于以往的手术有可能发生严重并发症和术后关节功能欠佳,而作的改进术式。方法　手术是作下颌骨患侧的升枝部截断,再将病变的髁状突摇摆松动折断,摘下离体进行修整成形后,再予以复位重建关节。此术式可保存部分的残留关节组织结构,便可保留关节特有滑动功能。结果　本组施行了5例6侧功能性颞下颌关节成形术,在术后半年以上作随访复查,张口咀嚼等功能都很良好。结论　在讨论中对颞下颌关节成形术的手术演变与手术安全进行了分析。对功能性颞下颌关节成形术的手术术式与手术命名作了评价。Objective A new method is reported for the functional arthroplasty of TMJ, compared with former ones that may accompanied by severe complication and poor functions.Method The operation procedures include amputation of the mandibular ramus of the affected side, shaking and fracturing of the affected condyloid process, exsomation and reshaping it, reduction and reconstruction of the TMJ. Part of TMJ tissues were reserved, and the particular sliding function of TMJ was preserved. Results 5 patients(6 sides) were treated with this method. Postoperative follow\|up demonstrated satisfactory results more than 6 months.Conclusion The development and security problem of arthroplasty are analyzed. The method and nomenclature of functional arthroplasty are also appraised in this report.
