根除幽门螺杆菌治疗功能性消化不良的疗效观察

摘　　要：目的探讨根除幽门螺杆菌(Helicobacter pylori,H.pylori)对H.pylori阳性的功能性消化不良(functional dyspepsia,FD)患者症状的治疗作用。方法 100例经胃镜检查及^(14)C-尿素呼气试验同时阳性的FD患者随机分为2组,对照组根据个体化原则给予抗抑郁、促动力药物(多潘立酮、氟哌噻吨美利曲辛片)治疗;治疗组加用根除H.pylori治疗,疗程1周。治疗后1个月和6个月用^(14)C-尿素呼气试验评估H.pylori。并进行症状评分。结果治疗组H.pylori根除率88.0%,其中H.pylori根除组1个月后和6个月后症状改善总有效率为63.6%和54.5%,与H.pylori未根除组(50.0%和16.7%)相比差异有统计学意义(P<0.01)。结论根除H.pylori可以在较长时问内改善FD症状,对于H.pylori阳性的FD建议根除Hpylori治疗。Objective To discuss the effect patients with functional dyspepsia （FD）. Methods of Helicobacter pylori （H. pylori） eradication in A total of 100 FD patients with H. pylori infection receiving both gastroendoscopy and ^14C - urea respiratory test were divided into therapy group and control group randomly. The control group was given individualized treatment including antidepressant and gastric motility promoting therapy. The therapy group was treated with H. pylori eradication based on individualized treatment. The course of therapy was 7 days. ^14C urea respiratory test was performed to evaluate the H. pylori eradication results one month and six months after therapy. For each patient, the symptoms before therapy and after therapy were assessed. Results The H. pylori eradication rate was 88% in therapy group . Of 63.6% and 45.4% patients with successful H. pylori eradication in the therapy group had a good symptomatic response one month and six months after the therapy respectively.There was significant difference between H. pylori eradication group and no eradication group （P 〈0. 01 ）. Conclusion H. pylori eradication can improve the symptoms of the FD patient. It is suggested that FD patients with H. pylori infection should receive the H. pylori eradication treatment.
