骨良性纤维组织细胞瘤2例报告并文献复习

摘　　要：目的探讨骨的良性纤维组织细胞瘤(benign fibrous histiocytoma,BFH)的临床病理特征、诊断及鉴别诊断要点与治疗方案。方法对2例发生于胫骨的BFH的临床表现、影像学特征、组织学形态进行分析,并复习相关文献。结果临床表现、CT及MRI图像与组织学特征均支持骨BFH的诊断,骨BFH并无特异性的免疫标记。结论 BFH在骨关节系统是一种鲜见的良性肿瘤,须密切结合临床资料、影像学特点及组织形态学特点,才能做出明确的病理诊断,并要与干骺端纤维缺损/非骨化性纤维瘤、骨巨细胞瘤等相鉴别。Objective To research the influencing factors in local recurrence and distant metastasis of rectal cancer after operation.Methods To review 412 profiles of rectal cancer patients,study 378 of which using rectal operation,and analyse 20 factors which might influence prognosis of postoperative rectal cancer by COX model.Result The operation excision rate was 91.7%（358/412）.The excision rate of radical treatment was 94.7%（358/378）.The 5-year survival rate was 47.6%（180/378）.By the end of 2006,in total 121 cases had developed recurrence,local recurrence was 47 cases,hepatic metastases were 41 cases,lung metastases 9 cases,bone meetastases 3 cases;10 cases had developed metastases in more than two organs,11 cases died of other causes.Conclusion There are 6 main influencing factors in local recurrence and distant metastasis of rectal cancer after operation： stage,primary tumor size,invasive depth,lymph node metastasis,pathological type,gross type.In addition,circumferential occupation by the tumor,distance between anus and lower margin of the tumor,and age also have some influence on prognosis.
