摘 要:目的探讨甲状腺乳头状癌(papillary thyroid carcinoma,PTC)HIF-1α、VEGF-C、D2-40表达及其相关性与临床病理意义。方法应用免疫组化SP法检测62例PTC组织中HIF-1α、VEGF-C、D2-40的表达,计数淋巴管密度(lymphatic vessel density,LVD),结合临床资料进行分析。结果 PTC组织内存在HIF-1α、VEGF-C的表达,与癌旁组织及腺瘤比较有显著性差异(P<0.05),淋巴结转移组表达率高于无淋巴结转移组,高分期组(Ⅲ期/Ⅳ期)瘤内HIF-1α、VEGF-C较低分期组(Ⅰ期/Ⅱ期)为高,两组比较有统计学差异(P<0.05)。HIF-1α表达与VEGF-C表达呈正相关(r=0.555,P<0.05)。PTC瘤周LVD较瘤内为高;瘤周LVD与临床病理特征有关,高分期组(Ⅲ期/Ⅳ期)的瘤周LVD较低分期组(Ⅰ期/Ⅱ期)为高,淋巴结转移组较无转移组为高,两组比较有统计学差异(P<0.05)。HIF-1α、VEGF-C表达与LVD成正相关(P<0.05)。结论 HIF-1α、VEGF-C在PTC癌组织中表达上调,二者表达呈正相关,与LVD也呈正相关,其异常表达参与了PTC的发生、发展,联合检测上述指标可能为甲状腺癌诊断、治疗、预后判断提供有力的临床指导。【Objective】 To investigate the expression with their correlations and signi-ficance of HIF-1α,VEGF-C,D2-40 in Papillary thyroid carcinoma(PTC).【Methods】 The expression of HIF-1α and VEGF-C was detected by SP immunohistochemical technique in 62 cases of PTC and LVD was counted with D2-40 antigen as markers of newborn lymphatic vessel.【Results】 HIF-1α and VEGF-C were expressed in PTC and there was significant difference compared with those in adenoma and tumor-around(P0.05).The expression was correlated with lymphatic metastasis and TNM stage.LVD of peritumor area of PTC was higher than that of intratumor region.LVD of PTC with lymphatic metastasis was higher than that of PTC without lymphatic metastasis,with statistical significance(P0.05).LVD of peritumor area of PTC was associated with TNM stage of PTC.LVD in peritumor area of PTC with high stage(stage Ⅲ and Ⅳ) was higher than that with low stage(stageⅠand Ⅱ),with statis-tical significance(P0.05).【Conclusion】 The expression of HIF-1α,VEGF-C in PTC was increased positively correlating between them,also with LVD.The abnormal ex-pression was probably involved in the occurrence and development of PTC and detection of these indicators combined could provide effective guidelines for clinical diagnosis,treatment and prognosis in PTC.
