β-半乳糖凝集素-3与上皮钙黏蛋白在甲状腺乳头状癌组织中的表达及其意义

摘　　要：目的:检测β-半乳糖凝集素-3和上皮钙黏蛋白在甲状腺乳头状癌组织中的表达,筛选甲状腺乳头状癌的辅助诊断指标。方法:采用组织芯片免疫组化法检测10例结节性甲状腺肿伴乳头状增生、10例甲状腺腺瘤、236例甲状腺乳头状癌组织中β-半乳糖凝集素-3和上皮钙黏蛋白的表达。结果:(1)β-半乳糖凝集素-3表达以甲状腺乳头状癌组最强,甲状腺乳头状增生组最弱,3组间差异有统计学意义(P<0.05);上皮钙黏蛋白表达以甲状腺乳头状增生组最强,甲状腺乳头状癌组最低,3组间差异有统计学意义(P<0.05);(2)β-半乳糖凝集素-3表达在甲状腺乳头状癌有和无淋巴结转移组间差异有统计学意义(P<0.05);上皮钙黏蛋白表达在甲状腺乳头状癌有和无包膜外侵犯及有和无淋巴结转移组间差异均有统计学意义(P<0.05)。结论:β-半乳糖凝集素-3蛋白表达阳性率在甲状腺乳头状癌伴淋巴结转移者高于无淋巴结转移者,上皮钙黏蛋白表达阳性率在甲状腺乳头状癌有腺体外侵犯及淋巴结转移病例低于无腺体外侵犯及淋巴结转移病例;联合测定β-半乳糖凝集素-3、上皮钙黏蛋白表达可判定有无腺体外侵犯及淋巴结转移。Objective： To examine the expression of galectin-3 and e-cadherin in thyroid papillary carcinoma so as to screen the auxiliary diagnostic index.Methods： Immunohistochemical SP method was used to examine the expression of galectin-3 and e-cadherin in 10 goiter with papillary hyperplasia,10 thyroid adenoma and 236 thyroid papillary carcinoma in tissue chips.Results：（1） The expression of galectin-3 peaked in thyroid papillary carcinoma,while declined greatly in the thyroid papillary hyperplasia,the difference being significant（P0.05）;the expression of e-cadherin was the highest in thyroid papillary hyperplasia and the lowest in the thyroid papillary carcinoma with a significant difference（P0.05）.（2） The expression of galectin-3 was the highest in thyroid papillary carcinoma with lymph node transferring and the lowest in thyroid papillary carcinoma without lymph node transferring,there was a significant difference（P0.05）;the expression of e-cadherin was the lowest in thyroid papillary carcinoma with lymph node transferring and extrathyroid invasion.The difference was also significant（P0.05）.Conclusion： The expression of galectin-3 is the highest in thyroid papillary carcinoma with lymph node transferring and the lowest in thyroid papillary carcinoma without lymph node transferring;the expression of e-cadherin is the lowest in thyroid papillary carcinoma with lymph node transferring or extrathyroid invasion and the highest in the thyroid papillary carcinoma without lymph node transferring or extrathyroid invasion.A combination of testing galectin-3 and e-cadherin is helpful in predicting lymphatic metastasis and extrathyroid invasion in papillary carcinoma of thyroids.
