血清cPSA、tPSA及C/TPSA比值在前列腺癌诊断中的应用

摘　　要：目的探讨血清复合态的PSA(complexed PSA,cPSA)、血清总PSA(total PSA,tPSA)及cPSA/tPSA(C/TPSA)比值对前列腺癌(prostatic cancer,PCa)与良性前列腺增生(benign prostatic hypertrophy,BPH)的鉴别诊断价值。方法对30例PCa(PCa组)患者和50例BPH(BPH组)患者采用Bayer公司的双抗体夹心磁微粒化学发光免疫法检测cPSA、tPSA的水平,并计算C/T PSA比值。应用ROC曲线图分析C/T PSA比值对PCa的诊断意义。结果 BPH组患者C/T PSA比值及血清cPSA、tPSA水平均显著低于PCa组(均P<0.01)。以0.82筛选界值时,敏感性为83.3%(25/30)。结论 cPSA、C/T PSA比值对PCa和BPH有重要的鉴别诊断价值。Objective To explore the value of serum total PSA（tPSA）,complexed PSA（cPSA） and cPSA/tPSA（C/T PSA） ratio in differential diagnosis of prostate cancer（PCa） and benign prostatic hyperplasia（BPH）.Methods The levels of tPSA and cPSA were determined in 30 PCa patients and 50 BPH patients by using double antibody sandwich magnetic particle chemiluminescent immunoassay,and the ratio of C/T PSA was calculated.The diagnostic significance of C/T PSA ratio was evaluated by ROC curve analysis.Results The levels of serum cPSA and tPSA and the ratio of C/T PSA in BPH patients were significantly lower than those in PCa patients（all P〈0.01）.A cut-off point of 0.82 detected 25 of the 30 PCa patients（83.3% sensitivity）.Conclusion The serum cPSA and C/T PSA ratio have significant values for differential diagnosis of PCa and BPH.
