转移相关基因CD44V6和人类乳头状瘤病毒(HPV)在喉癌中表达及意义

摘　　要：目的:探讨转移相关基因CD44V6和人类乳头状瘤病毒(HPV)在人喉癌组织中的表达及其临床意义。方法:采用免疫组织化学S-P法对60例喉癌、30例声带息肉、5例正常喉组织标本进行CD44V6及HPV表达情况检测。结果:CD44V6、HPV在喉癌组织中的表达阳性率分别为73.3%(44/60)和56.7%(34/60),显著高于声带息肉、正常喉组织(P<0.01);晚期(Ⅲ-Ⅳ期)、组织学分级2-3级(低分化)、伴颈淋巴结转移的喉癌组织中HPV、CD44V6阳性表达率均显著高于早期(Ⅰ-Ⅱ期)、组织学分级1级(高分化)、无颈淋巴结转移的喉癌组织(P<0.01)。喉癌术后3年或5年存活病例CD44V6表达阳性率明显低于未存活病例,而HPV的表达阳性率在术后3、5年存活或未存活病例间无统计学差异。HPV与CD44V6表达正相关(P<0.01)。结论:喉癌的发生、发展及预后与HPV、CD44V6表达密切相关。Objective：To study the expression and clinical significance of metastasis-related genes CD44V6 and human papilloma virus（HPV） in human laryngeal carcinoma.Methods： Detection of HPV and CD44V6 expression by using immunohistochemical S-P in 60 cases of laryngeal,30 cases of vocal cord polyps,5 cases of normal laryngeal tissue specimens.Results： Expression of CD44V6 and HPV in laryngeal carcinoma were 73.3%（44/60） and 56.7%（34/60） respectively,significantly higher than vocal polyps and normal laryngeal tissue（P0.01）;The positive expression of HPV,CD44V6 in advanced（stage Ⅲ-Ⅳ）,histological grade 2-3（low differentiation）,and with neck lymph nodes transfer of laryngeal cancer were significantly higher than early（stageⅠ-Ⅱ）,histological grade 1（high differentiation）,and no neck lymph nodes transfer（P0.01）.CD44V6 expression in the survivors of laryngeal cancer post-operative 3 or 5 years are significantly lower than the deceaseds,while the expression of HPV was not statistical different between survivors post-operative 3,5-year and the deceaseds.Expression of HPV was associated with CD44V6（P0.01）.Conclusion： The occurrence,development and prognosis of laryngeal carcinoma are closely related to expression of HPV,CD44V6.
