淋巴管生成调控机制与大肠癌临床病理特征的相关性

摘　　要：目的:研究大肠癌淋巴管生成的相关调控基因,以探寻其淋巴转移机制。方法:收集经手术切除的60例大肠癌组织及远癌切端肠组织标本,应用免疫组织化学技术及基因芯片技术,探寻与大肠癌淋巴管生成密切相关的基因,并分析与临床病理参数的相关性。结果:大肠癌组织和远癌切端肠组织间LVD(lymph vessel density)具有统计学差异(P<0.05),不同分化程度、浸润深度和淋巴结转移有无的大肠癌组织间LVD有显著差异(P<0.05),而在不同年龄、性别、组织学分型之间无明显差异(P>0.05);比较高淋巴管密度和低淋巴管密度大肠癌组织的基因芯片检测结果,有5个基因上调和11个基因下调5倍以上。结论:大肠癌组织的LVD显著高于远癌切端大肠组织,大肠癌组织的LVD与肿瘤分化程度、浸润深度及淋巴结转移有相关性,提示大肠癌组织中LVD随着原发肿瘤分化程度减低、浸润加深以及发生淋巴结转移而增高;高LVD的大肠癌组织与低LVD的大肠癌组织间存在差异表达的基因。Objective To study the related genes of regulating the lymphangiogenesis of colorectal carcinoma in order to explore the mechanism of lymph node metastasis.Methods Total 60 cases of colorectal carcinoma specimens and their surgical marginal tissue were collected and the related genes of regulating lymphangiogenesis in colorectal carcinoma were explored by immunohistochemistry and gene chip technology.The relationship between the gene expression and the clinicopathological parameters was analyzed Results There was a statistical difference in the lymph vessel density （LVD） between colorectal carcinoma specimens and surgical marginal tissue （P 0.05）.In colorectal carcinoma specimens,the LVD expression was significantly different in different tumor differentiation,invasive depth,and lymph node metastasis（P 0.05）,while no difference was found in ages,genders and histological types（P 0.05）.Comparasion of gene expression in high LVD carcinoma with that in low LVD carcinoma after gene chip showed that 5 genes up-regulated and 11 down-regulated more than 5 folds.Conclusion LVD is related to colorectal carcinoma invasion and metastasis.The expressions of LVD are different in the case with different tumor differentiation,invasive depth,and lymph node metastasis.The expression of LVD in colorectal carcinomas is increasing with the tumor differentiation worsened,invasion deepened and lymph node metastasis happened.There are different expression genes between high and low LVD colorectal carcinoma.
