摘　　要：目的观察人工肝血浆置换对肝衰竭患者凝血功能的影响及疗效。方法 30例肝衰竭患者在内科综合治疗的基础上予以血浆置换(plasma exchange,PE)治疗,在PE治疗前后检测血浆凝血酶原时间(prothrombin time,PT),活化部分凝血酶原时间(activated partial thromboplastin time,APTT)、血浆凝血酶时间(thrombin time,TT)、凝血酶原活动度(prothrombin activity,PTA)、血浆抗凝血酶原III(antithrombinⅢ,ATⅢ)、国际标准化比值(in-ternational normalized ratio,INR)、纤维蛋白原(fibrinogen,Fbg)及肝功能。结果患者治疗后PT、APTT、TT、PTA,AT-III及肝功能均有明显改善,临床症状及体征亦明显好转,总有效率80%,其中早期为93.75%、中期为80%、晚期为25%。结论 PE可显著改善早期肝衰竭患者的凝血及肝功能,缓解临床症状,提高存活率。Objective To observe the effect of artificial liver support system on coagulation indexes in patients with Liver failure.Methods Based on integrated therapy,30 patients with liver failure were also treated with plasma exchange（PE）.We compared the levels of PT,APTT,TT,PTA,AT-III,INR,Fbg and liver function before and after PE treatment.Results The indexes of PT,APTT,TT,PTA,AT-III and liver function were obviously improved after PE treatment.Conclusions PE can significantly improve the coagulation and liver function in patients with Liver failure.
