摘 要:目的 观察温热结合化疗药物及不同渗透压的灌注液对小鼠腹水瘤细胞生长的影响,探寻其达到最佳疗效时的灌注液介质。方法 建立昆明小鼠腹水瘤模型,在接种后的第1 天和第8 天,用丝裂霉素(MMC) 、不同温度、不同渗透压的灌注液或其联合应用,检测其对腹水瘤细胞的杀伤作用及毒副作用。结果 以上方法均能破坏小鼠腹水瘤细胞,减少瘤细胞数量,抑制腹水产生,延长小鼠生存时间,G0/G1 期肿瘤细胞百分率增加,S期肿瘤细胞百分率减少,瘤细胞DNA 指数(DI) 降低(P< 0 .05) ;实验组小鼠无术后肠粘连、腹腔内出血、肝肺转移及肝肾等脏器水肿。其中以43°C、重蒸馏水(DDW) 及丝裂霉素腹腔内灌注的方法效果最好。结论 低渗液,尤其是重蒸馏水结合温热化疗药物行腹腔灌注是抑制进展期消化道肿瘤发生腹膜种植转移的一种新的、有效的治疗方法,它可以防治胃肠道肿瘤患者术后发生腹腔内转移。Objective To evaluate the efficacy of combined intraperitoneal hyperthermic chemotherapy and fluid perfusion of varied hypotonicity on the growth inhibition of ascitic tumor cells in mice and to explore the most effective perfusion solution. Methods The model of ascitic tumor cells of KM mice was used. At the first and eighth day after the implantation of cancer cells, the killing and toxic effects on the ascitic tumor cells were systematically investigated after treated with MMC, varied temperature in hyperthermia, and varied hypotonic solution. Results Compared with the controls, the tumor cells in peritoneal cavity of KM mice decreased in number and ascites formation, the survival periods were prolonged, the percentage of tumor cells in G 0/G 1 phase increased, those in S phase and tumor cells DNA index (DI) all decreased ( P <0.05). No postoperative adhesion, no intraabdominal bleeding nor liver and lung metastasis were seen. However, the best treatment result was those received MMC, or hyperthermia at a temperature of 43 °C or DDW. Conclusion Hyperthemic and hypotonic solution, especially DDW containing chemotherapeutic agent by intraperitoneal infusion may be the treatment of choice in the management of peritoneal implantation or metastasis of advanced GI cancer. It may also be an effective therapeutic measure to prevent postoperative metastasis.
