食管癌患者手术前后细胞免疫的变化

摘　　要：目的　探讨手术切除对食管癌患者细胞免疫功能的影响。方法　随机抽取食管癌患者３４例，用碱性磷酸酶抗碱性磷酸酶（ＡＰＡＡＰ） 法，对其术前和术后各期Ｔ 细胞亚群进行自身对比研究，同时对２０ 例健康献血员进行了同期对比监测。结果　食管癌患者术前ＣＤ３ ＋ 、ＣＤ４＋ Ｔ 淋巴细胞、ＣＤ４ ＋／ＣＤ８ ＋ 及ＮＫ 细胞均明显低于正常对照组（Ｐ＜ ０．００１），术后前３ｄ ＣＤ３ ＋ 、ＣＤ４ ＋ 、ＣＤ８ ＋ Ｔ 淋巴细胞及ＮＫ细胞均降低明显（ Ｐ＜ ０．０５ ）；上述各项指标术后第５ｄ 开始恢复，以后各期除ＣＤ８ ＋ Ｔ 淋巴细胞外，与术前相比均无显著差异（Ｐ＞ ０．０５）。结论　食管癌患者术前免疫功能低下，术后细胞免疫功能降低更为显著，但可逐渐恢复。提示手术切除肿瘤组织后，患者免疫功能紊乱可以得到纠正。s Objective To reserch the effects of cellular immunity(CI) of patients undergone the resection of the esophageal carcinoma(EC).Method Tirty four EC cases were selected randomly,their T lymphocyts subsets were measured with alkaline phosphatase-anti-alkaline phosphatase(APAAP) method before and post operation day(POD). The numbers of T lymphocyte subsets and NK cells in blood donors were measured at the same time. Results The preoperative numbers of CD 3 +、CD 4 + T lymphocytes and NK cells and the ratio of CD 3 +/CD 4 + in EC patients were lower than that of control group (P<0.001). Compared with preoperation,the postoperative numbers of CD 3 +、 CD 4 +、 CD 8 + T lymphocytes and NK cells decreased obviously in earlier three days in EC patients (P<0.05),and they began to restore at 5POD. Conclusion EC patients have a lower CI function before operation and may further decrease after operation,but can restore gradually.It was indicated that the disturbence of immunity function of the patients can be regulated.
