早产儿视网膜病发生率及高危致病因素分析

摘　　要：目的探讨早产儿视网膜病变(retinopathy of prematurity,ROP)的发生率及危险因素。方法对2008年1月～2009年6月在新生儿科住院的244例孕周<36周,出生体重<2000g的早产儿生后4周行眼底检查。根据ROP国际诊断分期法诊断和分期。结果 244例患儿之中,有不同程度的ROP患儿有49例(其中ROPI期有37例患儿,Ⅱ期有10例患儿,Ⅲ期有2例患儿),ROP的发生率为20.08%。吸氧、出生体重、孕周、呼吸暂停、酸中毒、机械通气、PO2变化及感染、母亲妊娠并发症对ROP的发生具有明显影响;且孕周越短、出生体重越小,ROP发生率越高。结论 ROP的发生和吸氧、出生体重、孕周、呼吸暂停、酸中毒、机械通气、PO2变化及感染、母亲妊娠并发症等因素有关。孕周、出生体重、吸氧、呼吸暂停是ROP的独立危险因素。Objective To analyze the incidence and risk factors of retinopathy of prematurity（ROP）.Methods For 2008.1～2009.6 in our hospital neonatal gestational age were admitted 36 weeks,birth weight 2000 g of the 244 premature children 4 weeks after birth,fundus examination.ROP stage at diagnosis,according to international law for the diagnosis and staging.Results 244 patients with varying degrees of premature children with ROP in 49 cases（including 37 cases of ROP I,ROP Ⅱ of the 10 cases,ROP Ⅲ 2 cases）,ROP rate was 20.08%.Gestational age,birth weight,oxygen,mother pregnancy complications,acidosis,apnea,PO2 changes,mechanical ventilation and infection on the incidence of ROP has a significant effect;and shorter gestational age,birth weight,smaller,ROP incidence higher.Conclusion The incidence of ROP and gestational age,birth weight,oxygen,mother pregnancy complications,acidosis,apnea,PO2 changes and infection,mechanical ventilation and other factors.Gestational age,birth weight,oxygen,apnea is an independent risk factor for ROP.
