蛛网膜下腔出血动物模型的人工脑脊液置换疗法实验研究

摘　　要：目的　评估人工脑脊液（ＡＣＳＦ）置换疗法对蛛网膜下腔出血（ＳＡＨ）动物模型的治疗效果。方法研制一种含无机离子及其理化性质基本上与正常人体脑脊液（ＣＳＦ）相同的ＡＣＳＦ，采用小脑延髓池与腰池双穿刺等量引流技术置换ＣＳＦ，进行家兔ＳＡＨ治疗的实验研究。结果　正常家兔用ＡＣＳＦ置换后，其ＣＳＦ离子成分及ｐＨ 值仍在正常范围内；用生理盐水置换后，除钠离子外，其它离子浓度及ｐＨ 值都有极显著的变化（Ｐ＜ ０．００１）。ＳＡＨ家兔非置换组的生存率为５０％ ，ＳＡＨ家兔ＡＣＳＦ置换组及生理盐水置换组的生存率虽均提高至１００％ （Ｐ＜ ０．０５）；但ＡＣＳＦ置换组无不良反应，生理盐水置换组影响家兔的神经生理反射。结论　ＡＣＳＦ置换疗法可有效地治疗实验性ＳＡＨ，提高ＳＡＨ家兔的生存率。Objective To evaluate the therapeutic effect of replacement with artificial cerebrospinal fluid (ACSF) on the animal model of subarachnoid hemorrhage (SAH). Me thods ACSF was prepared in which the concentrations of inorganic ions as well as physical and chemical parameters were basically the same as the normal human cerebrospinal fluid (CSF). Normal CSF or CSF with blood was replaced equivalently by ACSF by means of double punctures from the cerebello medullary cistern and the lumbar cistern. The technique of ACSF replacement was used to treat rabbits with experimental SAH in this study. Results Contents of inorganic ions and pH of CSF in normal rabbits were in the normal range after the replacement with ACSF, on the contrary, contents of ions except Na +, and pH were markedly changed ( P <0.001) after replacement with physiological saline. The survival rate of SAH rabbits without any replacement was 50%, but the survival rates of SAH rabbits after ACSF or physiological saline replacement were increased to 100% ( P <0.05). No side effects were found in rabbits with ACSF replacement, however, an increase of muscle tone and an augmentation of spinal flexion reflex were displayed in rabbits with physiological saline replacement. Conclusion ACSF replacement therapy could treat the experimental SAH effectively, and raised the survival rate of SAH rabbits.
