False-positive Rate in Prenatal Diagnosis of Fetal Surgically Correctable Anomalies

摘  要:目的探讨产前超声诊断不同类型胎儿外科相关畸形的假阳性率及其临床意义。方法回顾性分析近3年来我院产前超声诊断胎儿外科相关畸形的资料,并进行产后或引产后追踪随访。结果经尸检或产后影像学及体格检查共成功随访胎儿外科相关畸形220例。胎儿胸腔病变中,产前超声诊断先天性膈疝、肺囊腺瘤、隔离肺、先天性心脏结构异常的假阳性率分别为0、8.33%、18.18%及2.83%;对胎儿盆、腹腔病变的检出中,十二指肠闭锁、小肠梗阻、食管闭锁、腹壁缺损、卵巢囊肿、骶尾部畸胎瘤的假阳性率分别为0、33.33%、28.57%、0、25.00%及0,而肢体畸形中,足内翻的假阳性率为11.11%。结论产前超声诊断胎儿某些外科相关畸形的假阳性率较高,临床应针对不同类型病变采取不同的诊疗措施及产前咨询。Objective To evaluate the false-positive rate(FPR) in prenatal diagnosis of different types of surgically correctable anomalies(SCA), and to verify its clinical value. Methods Clinical data of fetuses with US evidence of surgical anomalies were analyzed retrospectively. Postnatal follow-up was also conducted. Results 220 cases underwent complete follow-up. In prenatal diagnosis of thoracic anomalies, a percentage of FPR of 0 was seen in congenital diaphragmatic hernia, of 8.33% in congenital cystic adenomatoid malformation, of 18.18% in pulmonary sequestra- tion and 2.83% in cardiac structural anomalies. Concerning abdominal and pelvic malformations, duodenal atresia, small bowel obstruction, esophageal atresia, abdominal wall defects(Gastroschisis and omphalocele),ovarian cysts and saerococcygeal teratoma accounted for an FPR of 0, 33.33 %, 28.57 %, 0, 25.00% and. 0 respectively. Prenatal diagnosis of clubfoot resulted in 1(11. 11%)of 9 FPs. Conclusions The false-positive rates of some particular SCA diagnosed by prenatal ultrasound were probably high, in view of various malformations, different treatment and prenatal consultant should be applied in clinic.
