CK19和CEA mRNA表达与乳腺癌外周血微转移的相关性研究

摘　　要：目的检测乳腺癌患者外周血中细胞角蛋白19(CK19)和癌胚抗原(CEA)mRNA的表达,探讨其表达是否可作为判断外周血微转移及预后的分子生物学指标。方法采用逆转录-多聚酶链反应(RT-PCR)技术检测45例术前乳腺癌患者及30例乳腺良性病变患者外周血中CK19和CEA mRNA的表达情况。结果 CK19和CEA mRNA在乳腺癌患者血清中阳性率分别为62.2%和53.3%,在30例乳腺良性患者血清中CK19 mRNA阳性率为3.3%,无CEA mRNA表达。乳腺癌患者和良性肿瘤患者血清中CK19、CEA mRNA表达阳性率比较差异有统计学意义(P<0.01)。CK19和CEA mRNA阳性率与临床分期、淋巴结转移均有关,二者表达呈正相关(P<0.01)。结论乳腺癌患者外周血中CK19和CEA mRNA表达与临床分期密切相关,提示两项指标有望预测乳腺癌外周血微转移和判断预后。Objective To detect the expression of CK19 and CEA mRNA in peripheral blood of breast carcinoma and to discuss whether these levels can be used as molecular biological markers for predicting micrometastasis and evaluating prognosis. Methods Peripheral blood samples of 45 patients with breast carcinoma,30 patients with benign breast lesions were collected. CK19 mRNA and CEA mRNA were assessed by RT-PCR. Results The positive rate of CK19 and CEA mRNA in breast carcinoma patients was 62.2% and 53.3% and the related benign breast lesions were 3.3~. CEA mRNA was not found in benign breast lesions. Signifi- cant differences were found in the expression of CK19 mRNA and CEA mRNA in peripheral blood among patients with benign breast lesions and patients with breast carcinoma （P（0. 001）. The positive rates and related of CK19 and CEA mRNA were significantly correlated with clinical stage and lymph node metastasis （P〈0.01）. There was a close relationship between expression of CK19 and the expression of CEA mRNA （P〈0.05）. Conclusion CK19 mRNA and CEA mRNA can be detected in peripheral blood of breast carcinoma patients. The two markers can be used to predict micrometastasis and evaluate prognosis because they are closely correlated with clinical stage and survival.
