低分子肝素联合阿司匹林在短暂性脑缺血治疗中的临床疗效分析

摘　　要：[目的]探讨低分子肝素联合阿司匹林治疗短暂性脑缺血发作(TIA)的临床疗效。[方法]选择70例TIA患者随机分为对照组和观察组,两组均给予静脉滴注复方丹参、维脑路通、VitE、低分子右旋糖酐,口服阿司匹林100mg,1次/d,观察组加用低分子肝素5000u于脐旁皮下注射,q12,14d为一疗程。比较两组治疗前后血小板计数h(PLT)、凝血酶原时间(PT)、活化部分凝血活酶时间(APTT)、纤维蛋白原(Fg)的变化。[结果]观察组总体有效率为94.3%,明显高于对照组;观察组治疗后PLT、Fg明显降低(P﹤0.05),APTT、PT较治疗前延长(P﹤0.05),但仍未超过正常范围;对照组治疗后PLT、APTT、PT及与治疗前比较差异无统计学意义(P﹥0.05)。[结论]低分子肝素联合阿司匹林治疗TIA发作的疗效显著优于单用阿司匹林。[Objective]To investigate the effects of low molecular weight heparin and aspirin in transient ischemic attack.[Methods]70 transient ischemic attack patients were divided into control group and observer group,both groups give static drops compound danshen,vascular brain leads,VitE,low molecular dextran,oral aspirin 100mg,1 times/d,the observation group add with low molecular heparin in 5 000 u beside the q12 subcutaneously scanning. The 14 days were as one period of treatment. The two platelet count（before and after the treatment PLT）,prothrombin time（PT）,activate part blood coagulation time（APTT）live enzymes and fibrinogen（Fg）were compared before and after treatment.[Results]The observation group total effectiveness was significantly higher than those in the control group,with the total effective rate of 94.3%,The Fg and PLT of the observation group were significantly reduced（P﹤0.05）. APTT and PT were extended before treatment（P﹤0.05）,but still not beyond the normal range;The APTT,PLT and PT of the control group after treatment did not find significant difference with those before treatment（P﹥0.05）.[Conclusion]The efficacy of the low molecular heparin joint aspirin therapy in treatment of TIA is more effective than that of aspirin alone.
