悬吊运动疗法对50例颈源性眩晕患者的临床疗效观察

摘　　要：目的观察50例颈源性眩晕患者在使用悬吊运动疗法治疗后的疗效。方法来我院确诊的50例颈源性眩晕的患者,在同一治疗师指导下采用悬吊运动疗法(挪威redcord工作站)进行为期6周的颈部稳定性训练,侧重颈部整体肌、局部稳定肌的康复训练,并在治疗期间对患者进行健康宣教,在治疗前、治疗后6周、治疗后3个月由同一康复评定师分别对所有患者进行颈椎残障指数量表(neck disability index,NDI)和颈性眩晕症状与功能评估(evaluation scalefor cervical vertigo,ESCV),并在治疗结束后进行X线片及TCD复查。结果经过6周的悬吊运动疗法治疗后,50例患者的眩晕症状有不同程度的减轻,46例患者颈部疼痛及植物神经紊乱症状均有好转,查体:转颈试验(-),通过NDI和ESCV两项量表评分,两项评分各自在治疗前与治疗后6周、治疗后3个月比较发现:治疗6周后NDI和ESCV评分有明显统计学差异(P<0.01),治疗后3个月与治疗后6周无明显统计学差异(P>0.05)。45例患者X线片及22例患者TCD回报结果提示治疗前后未见改变。结论悬吊运动疗法作为一种新型的疗法,能很好地提高颈椎稳定性,改善颈部肌肉的功能从而起到治疗颈源性眩晕的作用,并能有效地预防颈性眩晕的复发。Objective To observe the curative effect of S-E-T （Sling Exercise Therapy） for CV （cervicalvertigo）. Methods 50 cases confirmed of cervical vertigo were trained for cervical stability in 6 weeks with the same physiotherapists with S-E-T （Norway redcord workstation） when given health education, especially the functional recovery of cervical global muscles and local muscles. All patients were recorded with NDI （Neck Disability Index） and evaluation scale for cervical vertigo in 6 weeks prior treatment and post-treatment and 3 months post-treatment by the same physiotherapists and examined X-ray and TCD again after the treatment. Results After the treatment of 6 weeks with S-E-T, cervical vertigo symptoms of 50 cases were relieved respectively and 46 cases with neck pain and autonomic nerve functional disturbance were obviously improved. PE： neck rotation test became negative. The scores of NDI and ESCV were compared respectively 6 weeks prior treatment and post-treatment and 3 monthspost-treatment, and the scores of NDI and ESCV were significantly statistical 6 weeks post-treatment compared with prior treatment （P〈0.01）. Compared with 3 months post-treatment and 6 weeks post-treatment, the scores of NDI and ESCV were not significantly statistical. （P〉0.05）. Conclusions S-E-T as the new method for the treatment of cervical vertigo can obviously strengthen the cervical vertebra stability, improve the function of neck, and eventually cure cervical vertigo. And it can prevent relapse effectively.
