MRA与动脉质子自旋标记联合对短暂性脑缺血预后评估的应用

摘　　要：目的评价磁共振血管造影（magnetic resonance angiography，MRA）及动脉质子自旋标记（arteiral spin labeling proton，ASL）联合对短暂性脑缺血发作（transient ischemic attack，TIA）预后评估价值。方法90例颈动脉TIA患者根据DWI表现分为单纯型TIA和暂时性症状合并梗死（transient symptoms associated with infarction，TSI型）2组。90例患者均分别行MRA及ASL检查，根据MRA及ASL表现分为4型：A型（MRA及ASL均异常）、B型（MRA及ASL均正常）、C型（MRA正常，ASL异常）、D型（MRA异常，ASL正常）。结果在A型表现里，单纯型TIA占20％，TSI型占80％；在B型的表现里，单纯型TIA占94．7％，TSI型占5．3％。A、B2型在2组间进行比较，差异有统计学意义（P〈0．01）。结论MRA及ASL联合检查可对TIA患者脑血管和脑灌注进行诊断，为临床对TIA患者预后评估提供影像学依据。Objective To evaluate the value of magnetic resonance angiography{MRA） combined with arteiral spin labeling proton （ASL）in evaluating the prognosis of transient ischemic attack（TIA}. Methods 90 patients with internal carotid artery transient ischemic attack {TIA） were devided into simplex type and transient symptoms associated with infarction（TSI） type based on the manifestations of DWI. All cases underwent MRA and ASL examination. TIA could be derided into 4 t3;pes according to the manifestations of MRA and ASL：type A （abnormal on both MRA and ASL）, type B （normal on both MRA and ASL）, type C Inormal MRA and abnormal ASL findings）, type D （abnormal MRA and normal ASL findings）. Results Simplex type accounted for 20% and TSI type accounted for 80% in type A; simplex type accounted for 94.7% and TSI type accounted for 5.3% in type B. type A and type B had statistically significant between each other （P〈0.01）. Conclusion MRA in combination with ASL is a good method in diagnosis of TIA, and which is of great significance in estimating the diagnosis and prognosis of TIA .
