小肠间质瘤MSCT征象与病理危险度分级的研究

摘　　要：目的探讨MSCT术前评估小肠间质瘤病理危险度的价值。方法回顾性对52例经手术病理证实的小肠间质瘤的MSCT资料进行分析，其中41例有增强扫描，并与病理结果进行对照。结果MSCT定位准确率100％。肿块最大横径1．6～22．5cm，平均值为10．5cm；肿瘤形态为类圆形15例、分叶或不规则形37例；边界清晰21例、边界不清31例；CT平扫密度均匀11例、不均匀41例，增强扫描轻度强化为6例、中度强化11例、显著强化35例；肿瘤与邻近血管关系无影响14例、血管推移22例、血管侵及16例；淋巴结转移0例，肝转移5例。低危组肿瘤的大小、形态、边界、密度同中、高危组比较有统计学差异。结论MSCT可对小肠间质瘤生物学行为进行恶性风险度的初步分级，为诊疗计划的制订和预后评估能提供重要信息。Objective To study the value of MSCT in evaluating the pathologic risk grades of small bowel stromal tumor preop- eratively. Methods The MSCT data in fifty-two cases （of 52 cases,41 cases underwent contrast-enhanced CT simulaneously） with small bowel stromal tumors pathologically confirmed were retrospectively analyzed and compared to the pathological results. Results The accurate rate in localizing the tumors by MSCT was 100%. The biggest transverse diameter of the tumor was 1.6-22.5 cm with average value of 10.5 cm;The tumors appeared as circle-like in 15 cases,lobulated or irregular in 37 cases, with definite boundary in 21 cases and indefinite boundary in 31 cases. On plain CT, the density of the tumors were homogeneous in 11 cases and unhomogeneous in 41 cases. After contrast medium administration,the tumors were slight enhancement in 6 cases, moderate enhancement in 11 cases and significant enhancement in 35 cases. The vessels nearby the tumors were no change in 14 cases,pushed in 22 cases and invaded in 16 cases. Lymphatic metastasis in 0 case and hepatic metastasis in 5 cases were detected. There were statistical significance between the lesions with low or moderate grade risk pathologically and the lesions with high grade risk pathologically in the size, shape, boundary and density of the lesions. Conclusion MSCT can be used to grade the malignant risk of small bowel stromal tumor which is important for the administration of treatment and prognosis assessment.
