摘 要:目的观察血糖波动与氧化应激指标的相关性,阐明平稳控制血糖的重要性。方法收集60例体检正常者及92例2型糖尿病患者(其中35例患者使用动态血糖监测系统监测血糖)空腹血清检测血糖、糖化血红蛋白(HbA1 c)、超氧化物歧化酶(SOD)、丙二醛(MDA)和8-异前列腺素F2α(8-iso-PGF2α)。结果正常人群与糖尿病患者血糖、HbA1 c及氧化应激指标比较,差异均有统计学意义(P<0.05);血糖、HbA1 c与SOD呈负相关(r值分别为-0.233和-0.166),与MDA、8-iso-PGF2α呈正相关(r值分别为0.375、0.169及0.382、0.170);血糖波动参数平均血糖漂移幅度(MAGE)、日内血糖平均绝对值(MODD)、血糖高于180 mg/dl的曲线下面积(AUC-180)与SOD呈负相关(r值分别为-0.404、-0.336、-0.376),与MDA、8-iso-PGF2α呈正相关(r值分别为0.560、0.438、0.414及0.504、0.587、0.583)。结论糖尿病患者血糖、HbA1 c水平、血糖波动指标均与氧化应激状态存在明显相关性。对糖尿病患者的血糖管理既要注重血糖水平的控制,更要关注血糖波动的影响因素。Objective To observe the correlation between blood glucose fluctuations and indicators of oxidative stress and to elucidate the importance of blood glucose control.Methods Sixty subjects with normal physical examination results and 92 patients with type 2 diabetes(35 of them were continuously monitored for blood glucose) were enrolled.Levels of glucose,glycated hemoglobin(HbA1c),superoxide dismutase(SOD),malondialdehyde(MDA),8-iso-prostaglandin F2α(8-iso-PGF2α) in fasting serum were detected.Results Blood glucose,HbA1c,oxidative stress indicators were significantly different between diabetic patients and normal controls(P0.05);blood glucose and HbA1c were negatively correlated with SOD(r=-0.233,-0.166);blood glucose and HbA1c were positively correlated with MDA and 8-iso-PGF2α(r=0.375,0.169;0.382,0.170);mean amplitude of glycemic excursions(MAGE),mean of daily differences(MODD),and area under the curve(AUC)-180 were negatively correlated with SOD(r=-0.404,-0.336,-0.376)and positively correlated with MDA and 8-iso-PGF2α(r=0.560,0.438,0.414;0.504,0.587,0.583).Conclusion Blood glucose levels,HbA1c levels,and blood glucose fluctuation indices are correlated with oxidative stress status.More attention should be paid to the impact of fluctuations in blood glucose besides the control of blood glucose levels in the management of blood glucose in diabetic patients.
