摘 要:目的探讨丘脑梗死临床症状的严重程度与血管狭窄部位、血流灌注、脑梗死危险因素及短期预后等方面的相关性。方法选择新发丘脑梗死患者33例,收集其临床资料,包括病史、临床表现及辅助检查等,并进行统计分析。结果 Logistic多因素相关分析结果显示,发病时临床症状的严重程度〔美国国立卫生研究院卒中量表(NIH-SS)评分〕与患者CT血流灌注(CTP)后循环的灌注减低有相关性(P=0.009),随着临床症状的加重,后循环区的灌注亦有减低。CT血管成像(CTA)前循环、后循环血管狭窄部位及CTP前循环灌注减低与临床症状无明显相关性(P=0.954,P=0.660,P=0.218)。结论丘脑梗死早期临床症状的严重程度与椎-基底动脉供血区的灌注减低有明显相关性,这一结果符合丘脑血供主要来源于后循环的特点,也提示血流灌注与临床症状的相符性较其他各项更准确,可为早期血管再通、逆转缺血争取时间,为判断临床预后提供依据。Objective To explore the correlations between severity of clinical synptoms and location of vascular stenosis,blood perfusion,risk factors of cerebral infarction,and short-term prognosis among thalamic infarction patients.Methods Clinical data including history,clinical manifestations,and auxiliary examination results of 33 patients with MRI-conformed acute thalamic infarctions were collected for statistical analysis.Results Logistic multivariate analysis showed that severity of the early clinical symptoms(NIHSS score) among these patients was correlated with the reduction of posterior circulation perfusion(P=0.009),the deterioration of which was noted with more severe clinical symptoms.No correlations were observed between severity and ETA stenosis of anterior and posterior circulation,and lower CTP perfusion of anterior circulation(P=0.954,P=0.660,P=0.218).Conclusion The severity of clinical symptoms in patients with thalamic infarction was correlated with the reduced perfusion of posterior circulation.This result agrees with the fact that blood supply of thalamus is mainly from the posterior circulation and suggests that blood perfusion is more accurate an indicator of synptoms which contributes to effective clinical treatment and provides basis for prognosis.
