行为矫正疗法促进慢性精神分裂症患者精神康复的疗效观察

摘　　要：目的探讨行为矫正疗法促进慢性精神分裂患者精神康复的临床疗效。方法在抗精神病药物治疗及护理的基础上,对60例入组病例进行3个月行为矫正疗法并配合正性强化措施,选用阴性症状量表(SANS)和护士用住院病人观察量表(NOSIE)进行行为矫正疗法前后比较。结果行为矫正疗法后SANS总分及各因子分均较行为矫正疗法前有显著差异(t=7.41,P<0.01);NOSIE中社会能力、社会兴趣、个人整洁、退缩4因子均较行为矫正疗法前有非常显著性差异(t=3.52,4.09,9.81,6.56;P<0.01);激惹因子较行为矫正前有所降低,且存在显著性差异(t=2.25,P<0.05);精神病表现及抑郁因子无明显差异(t=1.39,P>0.05)。结论行为矫正疗法配合正性强化措施,对改善慢性精神分裂患者懒散、孤僻、社会生活能力差的印象症状,提高社会适应能力,促进病人康复有积极作用。Objective To explore the clinical efficacy of behavior modification therapy on psychiatric rehabilitation promoting for the patients with schizophrenia.Methods In the basis of antipsychotic treatment and care,60 cases of patients enrolled were treated by behavior modification therapy,in coordination with positive strengthening measures for three months.The Scale for the Assessment of Negative Symptoms（SANS） and Nurses＇ Observation Scale for Inpatient Evaluation（NOSIE） were used to compare the differences before and after treatment.Results After treatment,the total score and each factors of the SANS showed significant differences with those before（t=7.41,P〈0.01）;as for NOSIE,significant differences were also found in social competence,social interest,personal cleanliness and withdrawal（t=3.52 4.09,9.81,6.56;P〈0.01）;irritation factor was decreased and significant difference occurred（t=2.25,P〈0.05）;no significant difference was found in mental performance and depression factor（t=1.39,P〈0.05）.Conclusion For patients with schizophrenia,behavior modification therapy with positive reinforcement measures has some positive effect on relieving the symptoms of chronic schizophrenic as laziness,loneliness and poor social life,improving the ability of social adaptation,promoting the rehabilitation and slowing down decline.
