摘 要:目的研究MHCⅡ类分子在肝细胞癌(HCC)组织中的细胞定位,探讨MHCⅡ类分子在HCC组织中的表达与术后复发的关系。方法采用双重免疫组化方法对术后3年内复发和不复发HCC患者的原发瘤组织石蜡切片染色,随机选取视野检测MHCⅡ分子表达,然后统计分析。结果 MHCⅡ分子在术后3年内不复发HCC组织的表达高于1~3年内复发组织(38.5%vs 12.2%,P<0.05),MHCⅡ分子在HCC组织中主要表达在浸润的巨噬细胞、淋巴细胞、树突状细胞(dendritic cell,DC)等。MHCⅡ分子表达与HCC患者的性别、年龄、肿瘤直径、肿瘤数目、有无包膜、门静脉癌栓、乙肝表面抗原、Edmonson分级等因素无明显相关(P>0.05)。结论 MHCⅡ分子与HCC的术后3年内是否复发风险密切相关,HCC组织中MHCⅡ分子高表达患者3年内不易复发。提示MHCⅡ分子可能可以作为HCC判断预后的分子指标之一。In order to find out the expression and cellular localization of MHC II molecule in hepatocellular carcinoma (HCC) tissue, and its relationship with postoperative recurrence of the HCC, we used double immunohistochemical paraffin section staining to detect the expression of MHC Ⅱ molecule in primary tumor tissue of HCC patients with or without 3-year postoperative recurrence. Then we selected visual field randomly and counted MHC Ⅱ molecules. The expression of MHC Ⅱ molecule was significantly higher in HCC without 3-year postoperative recurrence as compared with patients with postoperative recurrence (38.5% vs 12.2%, P 〈 0.05); MHC Ⅱ molecule expressed in HCC tissues mainly located in infiltrating macrophages, lymphocytes, dendritic cells (DC), etc. We also found that MHC II molecule expression did not obviously correlated to the HCC patients" gender, age, tumor size, tumor number, tumor envelope, tumor thrombus in portal vein, HBsAg and Edmonson grade, etc (P 〉 0.05). All the result demonstrates that MHC Ⅱ molecule is closely associated with postoperative recurrence risk of HCC patients within 3 years, and postoperative recurrence is occur seldom in HCC patients with high MHC Ⅱ molecule expression within 3 years. Thus, MHC Ⅱ molecules possibly can be used as a molecular marker for HCC independent prognostic factor.
