人工泪液联合性激素治疗绝经后妇女干眼的临床观察

摘　　要：目的比较两种方法治疗绝经后妇女干眼的临床效果。方法 137例绝经后干眼患者随机分为两组,观察组71例给予人工泪液治疗,对照组66例给予人工泪液联合性激素(含雌孕激素、雄激素之复合制剂)治疗,两组均定期随访6～12个月。结果自觉症状的缓解时间(半数患者),对照组31d,观察组44d,两组间差异有统计学意义;1年后基础泪液分泌试验显示有效(增加≥1mm):对照组58例(87.88%),观察组54例(76.06%);显效(增加≥3mm):对照组45例(68.18%),观察组39例(54.93%),两组间差异有统计学意义;1年后泪膜破裂时间检查显示有效(延长≥1s):对照组64例(96.97%),观察组60例(84.51%);显效(延长≥3s):对照组51例(77.27%),观察组41例(57.75%),两组间差异有统计学意义;1年后角膜荧光素钠染色评分显示有效(分值减少≥3分):对照组62例(93.94%),观察组57例(80.28%),两组间有统计学意义差异。结论人工泪液联合性激素(含雌、孕激素、雄激素之复合制剂)治疗绝经后干眼,其临床效果优于单纯用人工泪液治疗。Objective To compare the therapeutic efficacy of two methods of treating dry eye in post menopausal women.Methods 137 post menopausal women with dry eye were devided randomly into observation group（71 cases） and control group（66 cases）.Observation group accepted artifical dacryma treatment,control group accepted artifical dacryma combined with HRT.All patients followed up 6～12months posttherapy.Results There was marked difference of the time of relief of uncomfortableness（half patients） between the two groups.Follow 6 months,there were marked difference of Schirmer＇s test,Tear film brea-up time（BUT） and fluorescien score between the two groups.Conclusion For post menopausal female dry eye,artificial dacryma combined with HRT is better than solo artificial dacryma.
