摘　　要：[目的]通过对妊娠晚期不同程度地中海贫血胎儿大脑中动脉血流峰值速度middle cerebral artery peak systolic velocity（MCA-PSV）的分析,探讨妊娠晚期MCA-PSV快速筛查和预测重度地贫的价值,为临床提供一种弥补中孕产前诊断失败或错过的补救方法。[方法]回顾性分析2008.01~2010.12妊娠晚期接受MCA-PSV检查的病例,正常胎儿组100例,产前诊断确诊为各型地贫的病例25例,分析各类妊娠晚期地中海贫血胎儿大脑中动脉血流峰值速度的特点及MCA-PSV在胎儿地中海贫血的预测价值。[结果]重型地贫胎儿MCA-PSV明显高于正常组以及轻度和中间型地贫胎儿。以1.55MOM为切割值,晚孕重度地中海贫血的敏感性100%和特异性达100%。以﹥1.29MOM为切割值,中度地中海贫血的敏感性50%和特异性100%。[结论]妊娠晚期MCA-PSV具有快速筛查和预测重度地贫的价值,为临床提供一种弥补中孕产前诊断失败或错过的补救方法。[Objective]To investigate the quick prediction for fetal thalassemia in late pregnancy by middle cerebral artery peak systolic velocity in high risk systolic velocity.[Methods]100 cases of normal fetuses in control group and 25 cases of thalassemia fetus were included in the study.Compared the MCA-PSV in each group and calculated the prediction value.[Results]MCA-PSV in severe thalassaemia group was significantly higher than mild thalassaemia group.The sensitivity and specificity in predicting severe fetal anemia were 100% and 100% respectively and 50% and 100% in fetuses with moderate anemia.[Conclusion]Fetal MCA-PSV has the value of quick prediction.It can be a relied prediction method when the diagnosis is missed or failed.
