经乙状窦后入路显微手术切除大型听神经瘤28例临床体会

摘　　要：目的:探讨大型听神经瘤经枕下乙状窦后入路显微手术切除的临床疗效和手术技巧。方法:通过对28例经术后病理证实的大型听神经瘤显微手术治疗病例进行回顾性分析。结果:肿瘤全切除25例(89%),次全切除3例(11%)。面神经解剖保留23例(82%),听神经解剖保留7例(25%),死亡1例。结论:采用枕下乙状窦后入路显微手术切除大型听神经瘤,能有效地提高肿瘤全切率,并能较好保留颅神经功能。Objective：To study the clinical efficacy and surgical techniques of microsurgical resection of large acoustic neuromas via the suboccipital retrosigmoid approach.Methods：28 cases large acoustic neuromas pathologically confirmed by microsurgery surgery were analyzed retrospectively.Results：A complete tumor removal was achieved in 25 patients（89%） and 3 patients（11%） subtotal resection.The facial nerves were preserved anatomically in 23 patients（82%） and the acoustic nerves were preserved anatomically in 7 patients（25%）,one patient died.Conclusion：The rates of total tumor removal and reservation of cranial nerves can all be increased by suboccipital retrosigmoid approach for microsurgical removal of the large acoustic neuromas.
