摘 要:目的:研究夜磨牙症(sleep bruxim,SB)患者睡眠期咀嚼肌节律性运动(RMMA)的发生特征及其与睡眠周期的关系。方法:对30名SB患者、30名正常人进行连续2夜的多导睡眠监测,统计睡眠结构、睡眠效率、微觉醒指数以及磨牙指数。分析RMMA事件成群性,RMMA分布与睡眠周期的相关性。结果:SB患者微觉醒指数与正常对照相似(P=0.5641),且具有正常的睡眠效率及睡眠结构;但发生RMMA的频率约3倍于正常对照组(P<0.0001),并高发于睡眠周期的S1、S2期。2组监测对象男女之间微觉醒指数与磨牙指数的差异无统计学意义。SB相关的RMMA成群率达到75%,高于正常对照组的53%。56.4%SB患者的睡眠期,在RMMA发生后20 s与其发生前20 s时相比,睡眠时相发生了改变。孤立和成群出现RMMA时,睡眠时相改变的比例无统计学差异(P>0.05);RMMA后60 s夜磨牙患者睡眠时相发生改变的比例降至29.1%。结论:SB患者发生RMMA的频率约3倍于正常对照组,且高发于S2、S1期,虽约50%发生了睡眠时相的改变,但时间短暂,可迅速恢复。故SB患者具有正常的睡眠效率和睡眠结构。Objective: The aim of this study was to investigate the characteristics of rhythmic masticatory muscle activity (RMMA) episodes of sleep bruxism (SB) patients and assess the distribution of RMMA episodes in relation to sleep stage and sleep cycles. Methods: Two consecutive nights of polygraphic recordings were performed on thirty SB subjects and thirty healthy adults without SB participated as controls. Sleep architecture, sleep efficacy,micro arousal (MA) index and RMMA index were scored. RMMA/SB episode clustering and distribution of RMMA episodes in relation to sleep stage and sleep cycles were also analyzed. Results: SB subjects showed normal sleep architecture and sleep efficacy, but incidence of RMMA were higher than control subjects (P〈0.O001). RMMA/SB episodes occurred more frequently in stagel and stage 2. No difference was found between two groups in the micro-arousal index (P=0.5641). Within each study group, micro-arousal index and RMMA/SB index were not significantly different between genders. It was observed that 75% of all RMMA/SB episodes occurred in clusters, which was higher than those of the normal subjects (53%). With the RMMA/SB episode, the sleep stage shift occurred with 56.4% of episodes between B20 and A20. However, these values decreased to 29.1% after 60 seconds. There was no difference in the percentage of episodes associated with sleep stage shift for clusters (55.9%) or isolated RMMA/SB episodes (57.6%). Conclusion: SB subjects showed a higher incidence of RMMA than control subjects. Although half of episodes were associated with sleep stage shift, but the shift was transient. Our study showed that sleep bruxism (SB) subjects still have a normal sleep architecture and sleep efficacy.
