前牙镍铬合金烤瓷冠与二氧化锆全瓷冠修复的临床应用比较

摘　　要：目的比较镍铬合金烤瓷冠与二氧化锆全瓷冠修复前牙的美学效果及牙周组织变化。方法对80例患者的194颗前牙进行冠套修复,观察修复初和1年后修复体外形、色泽,以及牙龈变化,并进行比较。结果修复1年后,两种冠修复体的外形和色泽均无明显改变。镍铬合金烤瓷冠修复后,38%的(59颗)患牙牙龈缘灰线或黑线,34%的(53颗)患牙出现轻度牙龈炎;氧化锆全瓷冠修复后无1例患牙发生牙龈缘灰线或黑线,但有5颗患牙出现牙龈炎症。结论二氧化锆全瓷冠较镍铬合金烤瓷冠修复前牙美学效果更佳,且对牙周组织影响更小。Objective To compare the differences of aesthetic results and periodontal tissue changes between using nickel-chromium alloy-porcelain crown and zirconia all-ceramic crown to repair anterior tooth.Methods 194 anterior teeth of 80 patients were repaired,and the changes of the restoration shape,color and gum at the early stage and one year after the reparation were observed.The effects were compared between the two methods.Results In the observation,the crown shape and color of the two different materials had no significant change one year after repair.38% of Ni-Cr alloy-porcelain（59 crowns） displayed black or gray gingival line.34% of them（53 teeth） developed mild gingivitis.In contrast,no tooth manifested black or gray gingival line after using Zirconia all-ceramic crown,but 5 teeth got gingival inflammation.Conclusion Zirconia all-ceramic crown is not only significantly better than Ni-Cr alloy-porcelain crown in aesthetics,but also has less influence on the periodontal tissue.
