小梁切除术联合甲钴胺治疗晚期青光眼的疗效观察

摘　　要：目的：探讨小梁切除术联合甲钴胺治疗晚期青光眼的疗效。方法：选择46例晚期青光眼行小梁切除术,术中应用可拆除调整缝线,术后联合应用口服甲钴胺糖衣片0.5g tid,连续3~6个月。结果：术前眼压平均值为38.97＋2.91mmHg,术后眼压平均值为14.41＋2.17 mmHg。术后2周视力提高大于两行10眼（21.74%）,视力提高小于两行20眼（43.48%）,不提高14眼（30.43%）,减退2眼（4.34%）,追踪观察6~24个月,无视力丧失。视野改善8眼（17.39%）,视野不变34眼（73.91%）,视野缩窄4眼（8.70%）。结论：晚期青光眼通过小梁切除术,联合视神经保护剂的应用,可有效控制眼压,避免视力过早丧失。Objective：To investigate the effects of trabeculectomy combined with mecobalamon in treating late glaucoma.Methods：46 patients with late glaucoma were treated by trabeculectomy.Adjustable suture was applied.All patients received mecobalamin therapy after operation for 3 to 6 months.Results：Mean intraocular pressure（IOP） before and after operation was 38.97±2.91mmHg,and 14.41±2.17mmHg.In terms of vision change 2 weeks after operation： 10 eyes（21.74%） improved by more than two lines,20 eyes（43.48%） improved by less than two lines,14 eyes（30.43%） showed no improvement,and 2 eyes（4.34%） decreased.All 46 patients were followed-up for 6 to 24 monthes,no loss of vision ever found.In terms of visual field after operation：8 eyes（17.39%） improved,34 eyes（73.91%） had no change,and 4 eyes（8.70%） worsened.Conclusion：For late glaucoma,trabeculectormy combined with neuroprotective agents can effectively control IOP and slow down the loss of visual function.
