摘  要:目的了解深圳西乡医院近5年伤口分泌物中常见病原菌种类及大肠埃希菌(Escherichia coli,E.coli)耐药情况。方法收集2006年1月~2010年12月4 260例住院患者伤口分泌物,进行细菌分离、鉴定及药敏试验,用微量肉汤稀释法检测20种抗生素耐药情况;用双纸片法检测大肠埃希菌产超广谱β-内酰胺酶(ESBLS)情况。用WHONET5软件进行结果分析。结果伤口分泌物感染率最高的是大肠埃希氏菌,共检出1 210株。大肠埃希氏菌耐药率最高是氨苄青霉素和哌拉西林,其次是复方新诺明和四环素;大肠埃希氏菌对亚胺培南及美罗培南敏感率100%,哌拉西林/他唑巴坦与阿米卡星相对敏感。结论伤口分泌主要病原菌是大肠埃希氏菌,大肠埃希氏菌对常用抗生素多数不敏感,产ESBLS菌株往往表现多重耐药,应引起临床医生的高度重视。掌握我院细菌耐药情况,指导临床合理使用抗生素,有效控制耐药菌株产生。Objective To understand the bacterium species common pathogens in wound secretions and drug resistance of Escherichia coli.Methods From January 2006 December 2010 wound secretions were collected from 4 260 hospitalized patients and the common bacteria were detected and drug resistance of Escherichia coli to 20 antibiotics were tested.Results The wound infection rates of Escherichia coli was the highest and totally 1 210 strains were detected.Escherichia coli wa highly resistant to ampicillin penicillin and piperacillin,followed by trimetroprim and tetracycline;Escherichia coli waa 100% sensitibve to imipenem and realatively sensitive to Piperacillin/tazobactam and amikacin.Conclusions Escherichia coli is the main pathegens in wound secretions that less sensitive to antibiotics and attention be paid to clinical treatment in selection of drugs.
