摘　　要：目的:观察巩膜瓣可调整缝线联合前房注气术在原发性闭角型青光眼小梁切除术中的应用效果。方法:选择我院64例87眼原发性闭角型青光眼患者,根据房角检查须行滤过手术者,将其随机分为单纯小梁切除术组(对照组)31例43眼,巩膜瓣可调整缝线联合前房注气+小梁切除术组(观察组)33例44眼,比较两组术后1周低眼压和浅前房发生率,最佳矫正视力;术后3个月视野和晶体混浊程度;术后12个月眼压和滤过泡形态。结果:两组术后1周低眼压和浅前房发生率比较差异具有统计学意义(P<0.05);观察组术后3个月视野和晶体混浊程度较术前无进展征象;两组术后1周最佳矫正视力,术后12个月眼压和滤过泡形态比较差异无统计学意义(P>0.05)。结论:巩膜瓣可调整缝线联合前房注气术在原发性闭角型青光眼小梁切除术中应用简单有效、经济安全。Objective： To observe the clinical value of scleral flap adjustable suture and anterior chamber gas injection in trabeculectomy for primary angle-closure glaucoma.Methods：A total of 64 inpatients with 87 eyes diagnosed as primary angle-closure glaucoma were randomly divided into two groups： 31 patients with 43 eyes in control group were treated by trabeculectomy,while 33 patients with 44 eyes in observation group were treated by adjustable suture and anterior chamber gas injection and trabeculectomy.The ratios of hypotony and shallow anterior chamber,the best corrected visual acuity 1 week after operation,vision field and crystal turbidity 3 months after operation for 3 months,IOP and the filtering bleb morphology 12 months after operation were observed and compared between two groups.Results： After 1 week,there were significant differences in ratios of hypotony and shallow anterior chamber between two groups（P0.05）.After 3 months,there was no significant improvement in vision field and crystal turbidity.The best corrected visual acuity 1 week after operation,intraocular pressure and the filtering bleb morphology 12 months after operation were not statically significant different between two groups（P0.05）.Conclusion： Application of adjustable suture and anterior chamber gas injection with trabeculectomy is effective and simple in treating angle-closure glaucoma.
