NP方案时辰调节治疗非小细胞肺癌临床观察

摘　　要：目的比较长春瑞滨(NVB)和顺铂(DDP)组成的NP方案时辰化疗与常规化疗治疗非小细胞肺癌(NSCLC)的疗效和不良反应。方法 62例NSCLC患者随机分两组,时辰化疗组32例予NP方案时辰化疗,NVB 25 mg/m2加入生理盐水100 ml,17∶00点静脉滴注10 min,第1,8天;DDP 25mg/m2加入生理盐水500 ml,14∶00～18∶00,用德国产BodyGuard323型正弦曲线输液泵静脉滴注,第2～4天,给药峰值在16∶00点。常规化疗组30例,予NP方案上午时间用药。每例至少完成2周期化疗方评价疗效。结果时辰化疗组有效率为59.4%,常规化疗组有效率为33.3%,两组比较,差异有显著性(P<0.05)。主要不良反应是血液毒性、消化道反应、脱发;时辰化疗组Ⅱ度以上血细胞下降及恶心、呕吐较常规化疗组低(P<0.05)。结论采用NP方案时辰化疗治疗非小细胞肺癌疗效高于常规化疗,毒副反应明显小于常规化疗,在改善患者生活质量方面更具有优势,值得临床进一步研究。Objective To compare the clinical efficacy and adverse effects between chrono-chemotherapy with navelbine（NVB） combined with cisplatin（DDP） and conventional chemotherapy in the treatment of non small-cell lung cancer（NSCLC）.Methods Sixty-two patients with NSCLC were randomly divided into two groups： chrono-chemotherapy group and conventional chemotherapy group.Thirty-two patients in chrono-chemotherapy group were given NP chrono-chemotherapy regime： NVB 25 mg/m2 was diluted with normal saline to 100ml and intravenously infused at 17：00 for 10 min（days 1,8）;DDP 25 mg/m2 was diluted with normal saline to 500 ml and intravenously administrated from 14：00 to 18：00（days 2-4）,with the highest plasma drug level at 16：00.A sinusoidal infusion pump（BodyGuard323,Germany） was applied.Thirty cases in conventional chemotherapy group received NP regime in the morning.The efficacy was evaluated after chemotherapy was given repeatedly for at least 2 cycles.Results The effective rate in chrono-chemotherapy group was 59.4%,higher than that of conventional chemotherapy group（33.3%）.There was significant difference between the two groups（P0.05）.The main adverse effects included hematologic toxicity,gastrointestinal toxicity and hair loss.The occurrence of leucopenia above grade II,nausea and vomiting in chrono-chemotherapy group was lower than that in conventional chemotherapy group（P0.05）.Conclusion the NP chrono-chemotherapy regime for treating NSCLC had better therapeutic effect and fewer side effects than conventional chemotherapy,therefore,it has obvious advantages in improving patient′s life quality and is worthy of further study.
