全身伽玛刀治疗非小细胞肺癌的疗效分析

摘　　要：目的观察全身伽玛刀治疗非小细胞肺癌（NSCLC）的近期疗效、远期疗效及其并发症，探讨全身伽玛刀治疗非小细胞肺癌的应用价值。方法42例病理明确的Ⅰ-Ⅲ期非小细胞肺癌患者，应用OUR—QGD型体部伽玛刀治疗，治疗方案为50％～75％等剂量曲线4～8Gy／次，1次／d，每周5次，治疗4～10次，总照射剂量40～50Gy。结果总有效率（CR＋PR）为71．4％；1，2，3年生存率分别为95．5％，59．5％，33．3％。结论全身伽玛刀可提高肿瘤区的放射剂量，同时保护肿瘤周围正常组织，从而增加非小细胞肺癌的局部控制率、提高远期生存率、降低放射反应。Objective To evaluate the effect and complication of whole γ--knife stereotactic radiotherapy on the non-small cell lung cancer and explore the value of γ-knife stereotactic radiotherapy in treatment of non - small cell lung cancer. Methods 42 patients with non - small cell lung cancer：γ-knife radiotherapy patients,4 - 8Gy once a day, five times a week,4 - 10times and 40 - 50Gy in total. Results The overall response rate （ CR ＋ PR） in γ- knife was 71.4%. Thel-,2-and 3-year survival rates were 95.5% ,59.5% ,33.3%. Conclusion Compared with conventional radiation,γ-knife stereotactie radiotherapy could achieve tumor dose escalation, and give the normal tissue better protection at the same time. It could improve the tumor control rate of non-small cell lung cancer, improve long survival rate, and so could reduce the radiation reaction.
