肝癌患者血浆WIF基因甲基化检测临床研究

摘　　要：目的研究WIF－1基因在肝癌组织和血浆中的表达关系以及与肝癌发生、转移乃至复发的临床意义。方法收集南通大学附属肿瘤医院Z008年3月-2009年10月间48例肝细胞癌（HCC）、癌旁组织、对应血浆及正常健康体检者血浆标本24例，诊断标准以病理诊断为标准。采用甲基化特异性PCR方法对所有组织和血浆标本WIF—1基因表达进行分析．结果48例HCC患者临床标本中，25例WIF甲基化表达阳性，阳性率为52．08％，48例血浆中，15倒WIF甲基化表达阳性，阳性率为31．25％，组织扣血浆符合率为60％。癌旁组织以及健康对照组血浆中均未见WIF甲基化表达。WIF-1甲基化阳性与阴性患者中AFP、AFU表达均有明显差异（X^2=7．919，P〈0．05;X^2=9．0015，P〈0．05）；在与HBsAg关系中，发现WIF-1甲基化表达与HBsAg没有关系。15例血浆甲基化阳性的HCC患者中，10例复发或转移，占66．7％;33例非甲基化阳性者中，10例复发或转移，占30．3％，二者之间有明显的差异（X^2=5．61，P〈0．05）。结论甲基化阴性患者预后明显优于阳性患者，监测高危人群的肝癌组织和血浆中WIF—1基因表达对肝癌患者早期转移复发有一定的临床诊断价值，临床上血浆中WIF—1表达检测可以代替组织中WIF—1基因表迭，作为一种简便的方法易于临床推广应用，特别对AFP阴性的患者预后具有一定的临床价值。Objective To study the clinical value of the expression of the WIF-1 in plasma and tissue from hepatocellular carclnoma（HCC） patients. Methods A PCR was performed for the detection the expression of the WIF-1 gene in tissue and blood DNA from 48 cases of HCC patients. Results The expression of WIF-1 gene in HCC specimens was significantly higher than that in nonneoplastic liver tissue and normal liver tissues group（P〈0. 05）;Found that WIF-1 in plasma significantly diffrient in AFP （P〈0. 05） and AFU （P〈0. 05） ,but in HBsAg there was not significantly diffrient （P〉0.05）. After followed up, found the metastatic and recurrence rate were significantly different between WIF-1 （＋）group and WIF-1 （-） group in plasma （P〈0. 05）. Conclusion Detecting the expression of WIF-1 in plasma and tissue after hepatectomy of bepatocellular carcinoma may be contributed to the earlier auxiliary diagnosis of the invasiveness,metastasis and recrudescence. In clinic,detection of the expression of WIF-1 in tissue could be replaced by in plasma.
