摘 要:目的探讨免疫固定电泳技术在鏊定M蛋白上的应用。方法采用血清蛋白电泳技术、血清免疫固定技术(IFE)检测107例MM患者血清M蛋白。结果107例MM患者中97例血清蛋白电泳检出M蛋白,107例免疫固定电泳均检出M蛋白。M蛋白IFE分型结果为IgG型63例,其中IgG κ型29例,IgGλ型34例;IgA型25例,其中IgA κ型11例、IgAλ型14例;IgM型3例,其中IgMκ型1例,IgMλ型2例;单纯轻链型15倒,其中K型2例,X型13例。60例MM患者中有39例检出尿本-周氏蛋白,其中,12例为κ型,27例为λ型。三种方法中血清免疫固定电泳的检出率最高,血清蛋白电泳次之,尿液本周氏蛋白电泳检出率最低(P〈O.01)。结论采用免疫固定电泳技术,探作简单、快速,灵敏度高,沉淀带客易识别,是鉴定各类型M蛋白的较准确、直观的有效方法。Objective To study the application of immunofixation electrophoresis (IFE) in typing M proteins. Methods Serum M proteins were detected in 107 patients By serum protein electrophoresis and IFE. Results Among I07 patients with MM,96 of them were affirmed to had M band with serum protein electrophoresis but M bands appeared in all of the MM patients with IFE. IFE manifest:there were IgG 63 cases (IgG κ 29 and IgG λ 34),IgA 25 cases (IgA κ11 and IgAλ ), 14) ,IgM 3 cases (IgM κ 1 and IgM λ, 2) and free light chain 15 cases (r 2 and }, 13). Of 60 cases of MM,39 patients were detected to had Bence Jones proteins (12 κ and 27λ) in urine by IFE. The serum IFE had the highest sensitivity in identification M protein among the three methods. The second was serum protein electrophoresis and the lowest was the urine Bence Jones proteins (P ~ 0.01). Conclusion The technology of IFE, characterized by easy differentiation of electrophoretic bands ,simple operation,high sensitivity and rapidity,has great value in typing M proteins.
