摘 要:目的探讨广西地区宫颈上皮内瘤变(CIN)石蜡包埋组织中人类乳头状瘤病毒(HPV)分型检测的临床意义及基因芯片技术的应用。方法应用基因芯片技术分别对CINⅠ级28例、CINП级25例、CINШ级25例病理石蜡包埋组织标本进行HPV分型检测。结果 CINⅠ级HPV阳性率为46.4%(13/28),其中单一感染12例,三重感染(HPV16,33,59)1例;HPV亚型的感染频度依次为(前3位):HPV16(14.3%)、58(10.7%)和52(7.1%)。CINП级阳性率为64.0%(16/25),其中单一感染15例,双重感染(HPV11,18)1例;HPV亚型的感染频度依次为(前3位):HPV16(20.0%)、58(12.0%)和18(12.0%)。CINШ级HPV阳性率为96.0%(24/25),其中单一感染17例,双重感染7例(HPV16和58双重感染为主,共3例);HPV亚型的感染频度依次为(前3位):HPV16(52.0%)、58(28.0%)和33(16.0%)。结论 HPV16和58是广西地区各级CIN中最常见的感染亚型,HPV分型检测能够早期发现病原并且指导后续的治疗;基因芯片技术敏感性高,特异性强,可应用于病理石蜡包埋组织HPV分型检测。【Objective】 To investigate the application of genotype detection of HPV by DNA chips technology in paraffin-embedded specimens of cervical intraepithelial neoplasia and its clinical significance.【Methods】 Twenty-eight paraffin-embedded specimens of CIN Ⅰ,25 cases of CIN Ⅱ and 25 cases of CIN Ⅲ were tested for HPV genotypes using gene chips technology.【Results】 The prevalence of HPV infection was 46.4% in CIN Ⅰ specimens,with 12 cases being single infection,and 1 case being triple infection(HPV16,33,59).The three most common HPV genotypes were HPV16(14.3%),58(10.7%) and 52(7.1%),respectively.Sixteen positive cases(64.0%) were detected in 25 cases of CINⅡ specimens,with 15 cases being single infection,and 1 case being double infections(HPV11,18).HPV16(20.0%),58(12.0%) and 18(12.0%) were the first three most prevalent HPV genotypes,respectively.Twenty-four cases were positive(96.0%) in CIN Ⅲ specimens.Among these 25 patients,17 cases were single infection and 7 cases were double infections with HPV 16 and 58 being the most common double genotype infections.The first three most prevalent HPV genotypes were HPV16(52.0%),58(28.0%) and 33(16.0%),respectively.【Conclusions】 The most predominant genotypes in CIN are HPV16 and 58.HPV genotype detection in CIN may contribute to discover the virus in early stage of CIN and guide the subsequent treatment.Gene chips technology is a commendable approach to detect HPV genotypes in paraffin-embedded tissues due to its high sensitivity and specificity.
