原发性三叉神经痛微血管减压术后无效的临床分析

摘　　要：目的探讨微血管减压治疗原发性三叉神经痛无效的原阕。方法回顾分析本科1984年1月～2009年12月间微血管减压术后无效忠者29例的临床资料及再次手术探查情况。结果术后1周内疼痛未得到任何缓解。3月后，5例治愈，4例显效，2例有效；2例再次微血管减压者治愈；1例三叉神经松解术无效；15例感觉根部分切断术者，都有面部麻木，治愈7例，显效3例，有效4例，无效1例。术后效果良好，随访1年无复发。结论病程长、症状不典型的原发性三叉神经痛患者首次减压无效可在3个月后再次手术干预。Objective To explore the reason of failed microvascular decompression （MVD） for trigeminal neuralgia （TN）. Methods To investisate the clinical and repeat operation data in a consecutive series of 29 patients with failed microvascular decompression for TN between January 1984 and December 2009 in our institution retrospectively. Results After 1 week did not get any pain relief. Three months later, 5 patients were cured, 4 cases were cured, 2 cases were effective ; two cases were cured again, microvascular decompression; 1 case of trigeminal neurolysis invalided; 15 cases of patients who felt cut off the root part, have facial numbness, cured in 7 cases, effective in 3 cases, effective in 4 cases, 1 case. After good resuhs, follow - up of 1 year without recurrence. Conclusion TN of long course or atypical should be operated again after 3 months if the first operation failed.
