摘 要:目的研究右旋美托咪啶对丙泊酚靶控输注复合瑞芬太尼-七氟醚静吸全身麻醉深度的影响。方法 60例行择期全身麻醉下脊柱内固定手术患者分为对照组和试验组,每组30例。2组患者均采用丙泊酚靶控输注复合瑞芬太尼-七氟醚静吸全身麻醉,在稳定15 min后,试验组给予右旋美托咪啶0.5μg·kg-1,用生理盐水稀释至10 mL静脉泵注3 min;对照组给予生理盐水10 mL,静脉泵注3 min。记录给药即刻(T0)、给药后1(T1)、2(T2)、4(T4)、6(T6)、8(T8)、10(T10)和15 min(T15)时脑电双频指数(BIS)、听觉诱发电位指数(AAI)、Narcotrend指数(NI)、平均动脉压(MAP)及心率(HR)。结果对照组患者麻醉期间BIS、AAI、NI、MAP和HR均无明显变化(P>0.05),试验组患者麻醉期间AAI无明显变化(P>0.05)。试验组T6、T8、T10、T15时BIS低于T0时(P<0.05),T8、T10、T15时NI低于T0时(P<0.05),T2、T4、T6、T8、T10时MAP高于T0时(P<0.05),T2、T4、T6、T8、T10、T15时HR低于T0时(P<0.05)。试验组T6、T8、T10、T15时BIS低于对照组同时间点(P<0.05),T10、T15时NI低于对照组同时间点(P<0.05),T2、T4、T6、T8时MAP高于对照组同时间点(P<0.05)。结论丙泊酚靶控输注和瑞芬太尼-七氟醚静吸复合麻醉中,静脉输注小剂量右旋美托咪啶能进一步加深患者的麻醉程度。Objective To evaluate the effect of dexmedetomidine on the depth of anesthesia monitoring with target controlled infusion of propofol and combined anesthesia with remifentanil-sevoflurane. Methods Sixty patients undergoing selective spinal column internal fixationoperation under general anaesthesia were divided into experiment group and control group,thirty cases in each group.All patients were performed with target controlled infusion of propofol and combined anesthesia with remifentanil-sevoflurane.Fifteen minutes after anesthetic stabilization,the patients in experiment group were performed with dexmedetomidine 0.5 μg·kg-1(Dex was diluted with normal saline to 10 mL)by phleboclysis for three minutes,and the patients in control group were performed with normal saline 10 mL by phleboclysis for three minutes.The values of bispectral index(BIS),A-line ARX index(AAI),Narcotrend index(NI),mean arterial pressure(MAP) and heart rate(HR) were recorded at the moment of giving medicine(T0),1(T1),2(T2),4(T4),6(T6),8(T8),10(T10) and 15 minutes(T15). Results There was no significant change in BIS,AAI,NI,MAP and HR in control group(P0.05),and there was no significant change in AAI in experiment group during anaesthesia(P0.05).IN experiment group,compared with T0,the values of BIS were lower at the moment of T6,T8,T10 and T15(P0.05),the values of NI were lower at the moment of T8,T10 and T15(P0.05),the values of MAP were higher at the moment of T2,T4,T6,T8 and T10(P0.05),and the values of HR were lower at the moment of T4,T6,T8,T10 and T15(P0.05).Compared with control group,the values of BIS were lower at the moment of T6,T8,T10 and T15 in experiment group(P0.05),and the values of NI were lower at the moment of T10 and T15 in experiment group(P0.05),but the values of MAP were higher at the moment of T2,T4,T6 and T8(P0.05).
