上矢状窦及大脑镰旁脑膜瘤的手术治疗（附27例分析）

摘　　要：目的：探讨上矢状窦及大脑镰旁脑膜瘤的手术方法和治疗效果.方法：2000年3月至2010年10月,采用显微外科技术治疗上矢状窦及大脑镰旁脑膜瘤27例.结果：27例手术均顺利.依据Simpson分级标准,24例肿瘤全切除（ Simpson I-Ⅱ级）,3例次全切除（SimpsonⅢ级）.无手术死亡.术后均获随访3个月～8 a,25例恢复正常生活,2例复发（行二次手术后治愈）.结论：对上矢状窦及大脑镰旁脑膜瘤患者术前应充分研究其影像学资料,控制和减少术中出血,合理应用显微外科技术,肿瘤大多能全切除,同时减少术后神经功能障碍.Objective：To investigate the operative technique for the superior sagittal sinus and cerebral falx meningiomas and its curative effect. Methods ：The clinical data of 27 patients with superior sagittal sinus and cerebral falx meningiomas,who underwent microsurgical treatment in our hospital from March 2000 to Octo- ber 2010,were analyzed retrospectively. Results： The operation was successful in all the cases. According to Simpsong grade, 24 cases were totally resected , （Grade Ⅰ,Ⅱ of simpson system） ,3 cases were subtotally resec- ted, No patients died from the operation. During the follow up of 27 patients from 3 months to 8 years, 25 cases returned to normal life and the meningiomas recurred in 2 patients who received second operation. Conclusions ： Sufficient preoperation imaging evaluation , efficient management of operative bleeding and using microscope Surgical techniques in the resection of the superior sagittal sinus and cerebral falx meningiomas may improve the rate of removal of the tumors, and reduce postoperative nervous malfunction.
