前列腺癌斜坡转移1例报道并文献回顾  

Prostate cancer with clivus metastasis,A case report and review of literature

摘　　要：目的:探讨前列腺癌斜坡转移的发病特征、临床表现、治疗及预后。方法:回顾分析1例前列腺癌斜坡转移患者并复习文献。结果:国内尚无专门报道,搜集国外文献共得到16例患者,平均发病年龄64岁,以颅神经麻痹为首诊表现的占26.6%(n=4);前列腺癌疾病进展、出现斜坡转移的占73.3%(n=11);1例颅神经麻痹与前列腺癌几乎同时被确诊。从诊断为前列腺癌到发现颅神经麻痹的时间间隔为3.6个月到168个月,中位时间32.4个月。临床表现以第Ⅵ颅神经麻痹多见(n=8,57.1%)。联合应用手术、放化疗和内分泌治疗,大部分患者颅神经症状可得到缓解。出现颅神经症状后生存时间在2天到30个月,中位生存期6个月。结论:前列腺癌斜坡转移的早期临床症状隐匿,多在肿块进展对临近组织产生压迫引起相关颅神经症状时发现,及时给予神经减压治疗,神经症状可部分或完全缓解,但总体预后较差。Objective：To study the clinical features,treatment and outcomes of prostate cancer metastasis to clivus.Methods：A case of prostate cancer metastasis to clivus treated was investigated and related literatures were reviewed.Results： In literature review,there was no special report on prostate cancer metastasis to clivus in our country,16 patients were found in foreign literatures,and the mean age was 64 years.In 4 patients,cranial nerve palsy presented as the first sign of primary neoplasm,and later clivus metastasis from prostate cancer were 11 patients,there was only one patient whose cranial nerve palsy and the prostate cancer were diagnosed nearly at the same time.The median time from diagnosis of prostate cancer to the clivus lesion was 32.4 months,ranging from 3.6 to 168 months,and the median survival time after the cranial nerve palsy was 6 months,ranging from 2 days to 30 months.The most common symptom was the sixth nerve palsy（n=8,57.1%）.Neurological recovery were noted in most patients if the comprehensive therapy,such as surgery,radiotherapy,chemotherapy,endocrine therapy was given.Conclution： It is difficult to find the lesion early which is usual asymptomatic.The earlier the decompression is given,the more patients will get partial or complete resolution of cranial nerve symptoms,but the prognosis is poor.
