两段式种植系统采用非埋置术式疗效的对比研究

摘　　要：目的:对比两段式种植体(TP)I采用埋置和非埋置术式的疗效差异,并探讨TPI采用非埋置术式的可行性。方法:以95例因后牙区牙列缺损接受种植修复的患者为研究对象,共植入255颗内连接TPI。按种植体植入时的就位扭矩值(ITV)分组,ITV<30N.cm的147颗种植体分入A组(空白对照组),采用埋置术式;ITV≥30N.cm的108颗种植体随机分入B(52颗)、C(56颗)组,分别采用埋置、非埋置术式完成治疗。术后即刻拍摄口内根尖X线片确定种植体周骨吸收量(MBR)的基线,术后3个月完成修复,随访至少到术后12个月,计算累积成功率(CSR)和MBR。结果:术后3个月时,A、B组间MBR值无统计学差异,C组较A、B组MBR值高,且有统计学差异(P<0.05);术后12个月时,A、B、C三组间的MBR值无统计学差异(P>0.05);255颗种植体在观察期内,总体CSR为99.2%,A组种植体脱落2颗,12个月后的CSR为98.6%,B、C组没有种植体脱落,CRS均为100%。结论:两段式种植体采用非埋置术式简化了手术程序,减少了疼痛,观察期内的MBR和CSR与对照组没有统计学差异,值得临床推广。Objectives：To compare the outcomes of non-submerged surgery on two-piece implant（TPI）,and to analyze its feasibility.Methods：95 patients who missed teeth at the posterior region got 255 TPIs for restoration.3 groups are divided based on the implants＇ insertion torque value（ITV）.Group A,the controlled group,includes 147 implants using submerged surgery,ITV30N.cm.The rest 108 is divided into Group B（52 implants） and Group C（56 implants） radomly,using submerged and non-submerged surgery respecitvely,both ITV≥30N.cm.After surgenry marginal bone resorbasion（MBR） was immediately rated from intraoral radiographs as the bone＇s baseline;3 months later,implants supported restoration was finished and the follow-up was conducted at least 12 months after surgery to observe the MBR and to caculate the cumulated survival rate（CSR）.Results：No difference on MBR between group A and B 3 months after surgery,whereas a high MBR in group C was found and the difference was statistically significant.No difference on MBR among group A,B and C 12 months after surgery.The sum CSR was 99.2%.The CSR in group A is 98.6% for 2 lossed implants in this group,and 100% in group B and C.Conclusions：Non-submerged surgery on TPI simplifies the treatment procedure and diminishes the pain to patients,especially appears no difference on CSR and MBR within the observation period,be worthy to expanding the clinical usage.
