白内障手术联合房角分离治疗急性闭角型青光眼临床疗效观察

摘　　要：目的观察单纯白内障超声乳化术联合后房型人工晶状体植入术及房角分离治疗急性闭角型青光眼的疗效及手术指证方法回顾性分析2008年7月至2009年10月我院收治的急性闭角型青光眼34例(39只眼),所有患者均有不同程度晶状体混浊,入院后均行超声生物显微镜及房角镜检查见房角关闭<270,°给予药物控制眼压后行患眼透明角膜白内障超声乳化术联合人工晶状体植入术及房角分离术,术后跟踪随访10～16个月,平均12个月。结果 39只眼术后视力均有明显提高,35只眼眼压降至正常,4只眼联合应用0.5%噻吗心安滴眼液后眼压可降至正常。结论白内障手术联合粘弹剂行房角分离对部分急性闭角型青光眼疗效显著。Objective To study t he efficacy of the simple phacoemulsification extraction of cataract and intraocular lens（IOL） implantation and isolation of anterior chamber angle in therapy of primary angle closure glaucoma.Methods Retrospective study on 34 cases（39 eyes） of angle closure glaucoma,who were in hospital between July,2008 to October 2009.All of them had cataract.Them were examined with ultrasound biomicroscopy（UBM） and gonioscope,and gave the operation of the simple phacoemulsification extraction of cataract and intraocular lens（IOL） implantation and isolation of anterior chamber angle after their intraocular pressure（IOP） got normal.And then we had a follow-up about ten to sixteen months,twelve months in average.Results All of them had a obvious increase in their vision.The IOP decreased to normal for 35 eyes,and can be normal after using timolol for 4 eyes.Conclusion The simple phacoemulsification extraction of cataract and IOL implantation and isolation of anterior chamber angle is efficacy in therapy of primary angle closure glaucoma.
