三种封闭剂一次性治疗慢性有瘘型根周尖炎的临床疗效

摘　　要：目的比较3种根管封闭剂一次性充填慢性有瘘型根尖周炎术后疗效。方法对600颗有瘘型慢性根尖周炎单根管前牙随机分成3组进行一次性根管治疗,根管封闭剂分别采用氧化锌丁香油糊剂,Cortisomol糊剂,Endomethason ivory(美松)糊剂加牙胶尖进行冷侧压法充填,观察术后24 h和1周的反应。结果 3种根管封闭剂一次性根管充填术后24 h急性反应:氧化锌丁香油糊剂组与Cortisomol糊剂、Endomethason ivory糊剂2组差别有显著性,Cortisomol糊剂组与Endomethasonivory糊剂组差别无显著性;术后1周各组均无显著性差别,术后2周瘘管愈合,术后3个月根尖部炎症消除(X线片显示)。结论采用规范操作和优质根充材料,慢性有瘘型根周炎可以进行一次性根管治疗,Cortisomol糊剂和Endomethason ivory糊剂在目前临床根管治疗中较氧化锌糊剂更具优势,不仅减少了患者复诊次数,缩短治愈时间,同时也降低了术后24 h急性反应发生率,是较理想的根管充填材料。[Objective] To compare the clinical effect of three root canal sealers on root canal filling in one time for chronic periapical periodontitis with fistula. [Methods]600 single root canal teeth with chronic periapical fistula perodontitis were randomly divided into three groups.All of three groups were given one-time root canal therapy by lateral condensation,and the root canal sealers were zine oxide eugenol oil paste,Cortisomol paste and Endomethason ivory paste respectively.The effect was observed 24 hours and one week after treatment. [Results]There were significant differences in acute reaction at 24 hours after treatment between zine oxide eugenol oil paste and Cortisomol paste,Endomethason ivory paste,and the difference between Cortisomol paste and Endomethason ivory paste was not significant.There was no significant difference in effect at one week after treatment among three groups.The fistula had healed two weeks after treatment,and X-ray film showed that apical inflammation had disappeared three months after treatment. [Conclusion]With standard operation and high-quality root filling material,the one-time root canal therapy can be used in treatment of chronic periapical periodontitis with fistula.The effect of Cortisomol paste and Endomethason ivory paste is better than that of zine oxide eugenol oil paste.These two sealers can reduce the frequency of appointments,shorten the healing time,and reduce the incidence of acute reaction at 24 hours after treatment,which are the ideal root canal filling material.
