86例颅内蛛网膜囊肿的临床特征分析

摘　　要：【目的】探讨颅内蛛网膜囊肿的治疗方法及临床疗效。【方法】回顾性分析本院自2000年9月至2010年10月经头颅核磁共振和（或）CT确诊的86例患者的临床资料。其中28例定期观察，未做特殊治疗仅作一般处理；16例经保守治疗；共42例行手术治疗，其中21例行囊肿分流手术；17例行脑室镜下囊肿壁切除手术；4例行开颅囊肿壁切除术。【结果】随访6个月至10年经过治疗的患者治疗后，】盎床症状均有所改善，复查头颅MRI及CT显示手术治疗患者囊肿均有不同程度缩小。未做特殊者症状无明显变化，复查囊肿无明显增大。其中一例为右侧侧脑室枕角囊肿患者，在外院行囊肿切除手术，术后脑积水，来本院行分流手术，术后效果不佳，死亡。【结论】囊肿分流手术和脑室镜手术是治疗蛛网膜囊肿的首选治疗方案。[Objective] To explore the treatment method of intracranial araehnoid cysts（IACs） and their clinical effects. [Methods] The clinical data of 86 patients with IACs confirmed by cerebral MRI or CT were retrospectively analyzed in our hospital from Sep. 2000 to Oct. 2010. Among all the patients, 28 patients were followed up regularly and did not receive any treatment, and 16 patients were treated conservatively, and 42 patients underwent the surgery. Of the 42 patients with surgery, 21 patients underwent cyst shunting, and 17 patients underwent the resection of cyst wall under ventriculoscope, and 4 patients underwent craniotomy and resection of cyst wall. [Results] Clinical symptoms of all treated patients had been improved after the follow up from 3 months to 10 years. The reexamination of cerebral MRI or CT showed that the cysts of patients with surgery were shrunk at some extent. Patients who did not receive treatments had no obvious change in clinical symptoms, and their cysts also had no obvious augmentation. One patient diagnosed as arachnoid cyst in occipital born of right lateral ventricle received the cyst excision in other hospital, and then transferred to our hospital because of postoperative hydrocephalus and received shunt operation, and had bad results after operation, and finally died. [Conclusion] The cyst shunting and cyst excision under ventriculoscope may be the first choice for patients with IACs.
