老年低位直肠癌患者应用双吻合器行保肛根治术的临床研究

摘　　要：目的探讨双吻合器在老年低位直肠癌患者行保肛根治术中的临床应用。方法回顾性分析2002-07～2007-09间,在我院应用双吻合器行保肛根治术的72例年龄>65周岁的低位直肠癌患者的临床资料。结果无手术死亡,术后出现吻合口漏1例,吻合口狭窄7例,通过对症治疗均痊愈,全部病例1年后大便功能均正常;局部复发率10.8%,3年生存率78%。结论应用双吻合器行保肛根治术可扩大Dixon术的适应范围,明显提高老年低位直肠癌患者的术后生活质量。Objective To evaluate the application of double stapler for low rectal carcinoma to preserve rectal continence in elderly patients.Methods 60 cases of rectal carcinoma treated using double stapler in the past 5 years were retrospectively analyzed.Results There was no operative mortality,1 case suffering from anastomatic fistula and 7 cases from anastomotic stenosina,all these cases were recovery by symptomatic treatment.Defecation was under satisfactory control after 1 year.Local recurrence rate of anus-saving surgery was 10.8% and 3-year survival rates was 78%.Conclusion The indication of Dixon procedure for low rectal carcinoma could be broadened using the double stapler,thus it might improve the Elderly patients living quality with low-rectal carcinoma.
