摘  要:目的探讨外科切除非小细胞肺癌(non-small cell lung cancer,NSCLC)中蛋白基因产物9.5(protein gene product 9.5,PGP9.5)表达的临床病理特征和意义及其与神经内分泌分化标记物神经烯醇化酶(neuron specific enolase,NSE)、突触素(synaptophysin,SYN)表达的相关性。方法应用免疫组化SP法,检测PGP9.5、NSE、SYN在135例NSCLC组织中的表达。结果在135例NSCLC中有57例(42.22%)表达PGP9.5,在各种不同病理类型中,鳞癌、腺癌、大细胞癌其阳性表达率分别为61.11%、31.48%2、5.93%,差别有统计学意义(P<0.05)。NSCLC中PGP9.5的表达与NSE(P<0.05)、SYN(P<0.005)表达相关,差别有统计学意义。也与肿瘤的淋巴结转移相关,差别有统计学意义(P<0.05),而与肿瘤TNM分期及预后无关(P均>0.05)。结论非小细胞肺癌中PGP9.5与SYN、NSE的表达呈正相关关系,三者的联合检测应用有助于NSCLC神经内分泌分化的判断。在不同病理类型的非小细胞肺癌中PGP9.5表达存在差异,其中鳞癌的表达率最高。PGP9.5表达阳性的NSCLC更容易出现淋巴结的转移,但该表达并非是判断其预后的独立因素,仅仅是促进肿瘤进展众多生物学事件之一。Objective To investigate the clinicopathologic features and the significance of protein gene product 9.5 ( PGP9. 5 ) in surgically resected non-small cell lung cancer ( NSCLC ) and to determine the relationship between the expression of PGP9.5 and neuron specific enolase (NSE) and synaptophysin (SYN). Methods Immunohistochemical staining for PGPg. 5, NSE and SYN was performed on paraffin embedded sections from 135 NSCLC patients who had been treated with complete curative surgery. The results of immunohistochemical staining, clinicopathologic parameters, and follow-up data were analyzed. Results Immunophenotype for PGP9.5 was positive in 57 of 135(42.22% ) NSCLCs. The positive expression of PGP9.5 was 61.11% in squamous cell carcinoma,31.48% in adenomatosum carcinoma,25.93% in large cell carcinoma( P 〈 0. 05 ). The results of staining for PGP9.5 was related with lymph node me tastasis, and also with the expression of NSE ( P 〈 0.05 ) and SYN ( P 〈 0. 005 ) but was not cor-related with TNM stage and survival time. Conclusion The expression of PGP9.5 was positively correlated with the expression of NSE and SYN in NSCLC. Combined analysis of PGP9.5, NSE and SYN can provide valuable evidence for diagnosis of neuroendocrine differentiation in NSCLC. The frequency of PGP9.5 expression was significantly higher in squamous cell carcinoma, compared with the other histological subtypes. The expression of PGP9.5 is specifically related with lymph node metastasis. PGP9.5 expression is independent progmostic factors. It is only one of numerous biological events that promote tumor progression.
