超前镇痛对阻生齿拔除患者心理影响研究

摘　　要：目的:探讨采用超前镇痛对阻生齿拔出患者心理的影响。方法:120名阻生齿患者随机分为研究组及对照组,每组各60例。研究组给予超前镇痛,采用VAS疼痛评分量表、HAMA焦虑量及HRDS抑郁量表,观察两组患者就诊时、术前及术后第1天、第7天的疼痛及心理变化。结果:就诊时两组患者VAS、HAMA、HRDS评分比较,差异无统计学意义(P>0.05),对照组术前VAS、HAMA评分[(3.40±0.85)分、(11.20±2.57)分]高于研究组[(0.50±0.04)分、(9.40±2.19)分],差异有统计学意义(t=5.74,P=0.009;t=3.97,P=0.017);对照组术后第1天VAS、HAMA评分[(2.90±0.47)分、(8.20±2.24)分]高于研究组[(0.30±0.05)分、(5.30±1.36)分],差异有统计学意义(t=4.28,P=0.010;t=4.06,P=0.012);术前和术后第1天两组HRDS评分比较,差异无统计学意义(P>0.05),术后第7天两组患者VAS、HAMA及HRDS评分比较,差异无统计学意义(P>0.05)。结论:阻生齿拔除患者超前镇痛能够减轻患者的焦虑心理,有利于维护阻生齿患者围术期身心健康。Objective： To investigate the effect of advanced analgesia to psychological on patients with impaction exelcy- mosis. Methods： 120 patients with impaction tooth were divided into control group and research group, each 60 patients. Advanced analgesia were given to research group before operation, change of ache and psychological were evaluated by visual analogue scale （VAS）, Hamilton anxiety scale （HAMA） and Hamilton depression scale （HRDS） in visiting time, before operations and 1 and 7 days after operations. Results： There was no difference of VAS, HAMA and HRDS scores between control group and research group （P〉0.05）. Before operations, scores of VAS and HAMA in control group [（3.40±0.85） points, （11.20±2.57） points] were higher than those of research group [（0.50±0.04） points, （9.40±2.19） points], and there were significant differences between them （t=5.74, P=0.009; t=3.97, P=0.017）. ]day after operation, score of VAS and HAMA in control group [（2.90±0.47） points, （8.20±2.24） points] were higher than those of research group [（0.30±0.05） points, （5.30± 1.36） points], and there were significant differences between them （t=4.28, P=0.010; t=4.06, P=0.012）. There was no difference of HRDS score between before and 1 days after operation （P〉0.05）. There was no difference of VAS, HAMA and HRDS scores between control group and research group 7 days after operation （P〉0.05）. Conclusion： Advanced analgesia can decrease psychological anxiety in patients with impaction exelcymosis, and is helpful to maintenance physical and mental health.
