摘 要:目的探讨不同程度高度近视眼视网膜电图明视负向反应(PhNR)的改变。方法高度近视眼患者20例(40眼)分为高度近视1组(-6.00^-10.00 DS)10例(20眼)和高度近视2组(-10.25^-20.00 DS)10例(20眼),同时选取正常成人10人(20眼)为对照组,应用重庆康华AVE8000视觉电生理仪检查PhNR。结果对照组受试者在强度1(2.000e-1 cd.s/m2)刺激下振幅较强度2(2.000e-2 cd.s/m2)和强度3(2.000e-3 cd.s/m2)明显升高,差异有统计学意义(P<0.05);三种刺激光强度下潜伏期比较,差异无统计学意义(P>0.05)。采用最佳刺激强度(2.000e-1 cd.s/m2)对高度近视1组和2组患者进行PhNR测试,高度近视1组与正常组振幅比较,差异无统计学意义(P>0.05);高度近视1组和对照组振幅较高度近视2组均明显升高,差异有统计学意义(P<0.05);3组间潜伏期比较,差异无统计学意义(P>0.05)。结论随着高度近视度数增加,其视网膜内层功能也受到损伤,PhNR可以作为高度近视眼内层视网膜功能检测的一个视功能评价指标。Objective To evaluate the changes of PhNR in patients with high myopia (HM) of different diop- ters. Methods Fourty eyes (20 cases) with HM were divided into HM group1 ( -6.00 ~ -10.00 DS) and HM group 2 ( - 10. 25 ~ -20.00 DS) . Twenty eyes ( 10 cases) with normal vision were enrolled as the control group. Photopic negative re- sponses (PhNR) were evaluated using AVE8000 examiner of vision electrophysiology. Results The amplitudes of the control group increased notably in response to stimulus intensity No. 1 (2. 000e - 1 cd ~ s/mz ) compared with intensity No. 2 ( 2. 000e - 2 cd · s/mz ) and intensity No. 3 ( 2. 000e - 3 cd ·s/m2 ) with statistical significance ( P 〈 0. 05 ) . Lateneies of the three stimulus intensity displayed no significant differences (P 〉 0. 05 ) . HM group 1 and HM group 2 were examined with optimized stimulation intensity. No significant difference was noted in amplitudes between HM groupl and the control group ( P 〉 0. 05 ) .The amplitudes of HM group 1 and control group remarkably increased in comparison with HM group 2 with statistical significance (P 〈 0. 05 ) . No significant difference in latency was noted among the three groups ( P 〉 0. 05 ) . Conclusion With the in- crease of diopters in high myopia, functional damage of the inner retina advances as well. PhNR examination provides may serve as a visual function evaluating index for inner retina in high myopia.
