利培酮口腔崩解片与氯氮平治疗112例精神分裂症对照研究

摘　　要：目的:探讨利培酮口腔崩解片治疗精神分裂症的疗效和安全性。方法:112例符合《中国精神障碍分类与诊断标准》第3版(CCMD-3)的精神分裂症患者随机分为两组,分别给予利培酮口腔崩解片(研究组,n=56)和氯氮平(对照组,n=56)治疗,疗程8周。分别于治疗前及治疗第1、2、4、6、8周末采用阳性和阴性症状量表(PANSS)和不良反应量表(TESS)评定临床疗效和不良反应。结果:利培酮口腔崩解片组显效率76.8%,有效率94.7%。氯氮平组显效率75.0%,有效率91.1%。两组间显效率有效率的差异无统计学意义。两组间治疗后各时点PANSS总分、阳性症状、阴性症状以及一般病理症状各项分值均低于治疗前,利培酮口腔崩解片对于阴性症状的治疗优于氯氮平。利培酮口腔崩解片组的嗜睡、便秘、头疼头晕、乏力、流涎、心动过速、记忆力下降的发生率明显低于氯氮平组,主要不良反应为锥体外系反应。结论:利培酮口腔崩解片治疗精神分裂症疗效确切,服用方便,是一种安全有效的抗精神病药物。Objective ： To explore the efficacy and safety of domestic risperidone orally disintegrating tablets in the treatment of schizophrenia. Methods： 112 patients met the criteria of Chinese classification of mental disorders ,3rd edition （CCMD -3 ） for schizophrenia were randomly assigned to two groups, were given risperidone orally disintegrating tablets （ study group, n = 56 ） and clozapine （ the control group, n = 56） treated for 8 weeks, before treatment and the end of], 2,4,6,8 week after treatment. The positive and negative syndrome scale （PANSS） and side reaction scale （TESS）. assessed the clinical efficacy and adverse reactions. Results： The responsive rate and effective rate were 76.8% and 92.8% respectively in risperidone orally disintegrating tablets group and 75. 2% and 92.3 % in Clozapine group without significant difference between the two groups. The total and factor scores of PANSS were significantly lower after treatment in both groups, but there were no significant difference between the two groups. The incidence of drowsiness, constipation, dizziness headache, weakness, salivation, tachycardia memory lossin Risperidone orally disintegrating tablets group was were significantly lower than the clozapine group and were statistically significant differences between the two groups . the main side effects of risperidone orally disintegrating tablets was EPS. Conclusions： Domestic risperidone orally disintegrating tablets is effective in the treatment of schizophrenia, fewer adverse reactions and is conducive to patients compliance to treatment is a safe and effective antipsychotics.
